Achieving Natural Nose Height and Improved Rhinitis: A Case Study After Filler Removal

Hello, this is Dr. Cha-Young Kang, Director of Nose Lab Clinic.


Today, I’m sharing a case where a patient underwent surgery to address persistent rhinitis symptoms while achieving a naturally balanced nasal line. This case highlights the combination of functional and aesthetic rhinoplasty for optimal results.

1. Surgical History and Current Symptoms


Surgical History:

  • 8 years ago: Rhinitis surgery

  • 4 years ago: Nasal filler injection followed by dissolution


Current Symptoms:

  • Alternating nasal congestion during sleep

  • Frequent throat mucus buildup

  • Post-nasal drip

2. Initial Examination


  • Frontal view: Flat and wide nasal bridge with a slight deviation.

  • Side view: Low nasion (bridge start) and a droopy nasal tip, contributing to a protruded mouth appearance.

  • Oblique view: Low bridge and a downward-pointing nasal tip.

  • Nasal Base View: Asymmetrical nostrils with a bulbous tip.


3. Patient’s Goals


  • Achieve a natural nose height without excessive prominence.

  • Correct the nasal deviation for a straighter appearance.

  • Harmonize nasal structure with overall facial proportions.

  • Open to using silicone for nasal augmentation.


4. Surgical Plan and Execution

  • Reduction of turbinate size via rhinitis surgery to improve airflow.

  • Septoplasty to correct septal deviation and expand nasal passages.

  • Enhanced nasal cavity space for improved breathing.

  • Creation of a natural and smooth nasal dorsum using silicone implants.

  • Adjustment of nasal tip projection and rotation to soften a protruded mouth appearance.

  • Sculpted the nasal tip for a refined and elegant shape.

5. Results

  • Slimmed and streamlined nasal bridge.

  • Straighter and more aligned nasal shape.

  • Enhanced facial harmony with subtle three-dimensional contours.


b. Side View

  • Appropriately elevated nasal bridge starting at the nasion.

  • Improved nasolabial angle for a softer and more proportional profile.

  • Lifted nasal tip to eliminate droopiness.


c. Oblique View

  • Smooth and natural nasal bridge transition.

  • Optimized tip position for balanced projection and rotation.


d. Nasal Base View

  • Symmetrical nostrils with a proportional appearance.

  • Improved nasal tip definition and stability.

  • Restored breathing function with unobstructed airflow.

6. Expert’s Perspective


This case required a delicate balance between functional correction and aesthetic enhancement. The patient’s desire for a natural-looking result with moderate nasal height was meticulously achieved. Addressing the rhinitis symptoms significantly improved the patient’s quality of life, while the nasal design brought subtle yet impactful changes to their overall appearance.
