1. Preoperative Design
This patient came to my clinic to correct a blunt and deviated nose. The skin was thick and the implant was bent to the right side, the nose tip had a lot of scar tissue in the right which made it look even more bent. The columella had also thicken and made the nostrils to narrow significantly.
2. A surgical plan
The plan is to remove enough scar tissue from the nose tip, improve the blunt appearance, and to improve the asymmetry of the nasal bone by performing an lateral osteotomy to place the bent-looking parts of the silicon implant in the right center location. To improve the scarring of the columella, the plan was to transplant dermis and cartilage in the lower part of the columella.

4. A general Overview
Bulbous nose correction: 1. Removal of the subcutaneous soft tissue 2. Cartilage relocation 3. Giving a three-dimensional effect to increase the height of the nose tip. This patient had developed excessive scar tissue in the nose tip, so an adequate amount of scar tissue was removed, and the damaged cartilage of the nose tip alar was reconstructed and relocated. The self-rib cartilage was used to increase the height of the nose tip.
