Hello, this is Dr. Cha-Young Kang from Nose Lab Clinic.


Today, I’m sharing the story of a patient who underwent bulbous nose correction and short nose refinement to achieve a more elegant, defined look with enhanced facial harmony.

1. Initial Evaluation


a. Frontal View

  • Severe bulbous nose

  • Retracted columella (columellar retraction)

  • Low radix (starting point between the eyebrows)

  • Short nasal length

  • Low radix and nasal dorsum

  • Blunt, under-projected tip

  • Upturned nose

  • Retracted columella

  • Mild mouth protrusion (relative to the nose)

3. Surgical Approach


a. Columella Reconstruction

  • Advancement of the septal cartilage to stabilize columella position

  • Rib cartilage graft (autologous) for structural support and elongation

  • Restoration of columella retraction for a more harmonized nasal projection


b. Bulbous Nose Correction

  • Removal of excess soft tissue beneath nasal skin

  • Repositioning and suturing of alar cartilage (tip-plasty)

  • Nasal tip projection enhancement


c. Shape Refinement

  • Adjusting nasal tip height for better definition

  • Radix augmentation for a natural transition from the forehead to the nose

  • Overall balance improvement

This case required comprehensive corrections, as the patient had both a bulbous nose and a short nose with columellar retraction. By advancing the septal cartilage and utilizing rib cartilage grafts, we achieved stability and enhanced projection. Additionally, alar cartilage restructuring and soft tissue reduction contributed to a more refined, elegant look.


Most importantly, the enhanced tip projection and balanced proportions gave the patient a more sophisticated and graceful appearance.
