This case presents a patient who visited NoseLab Clinic with nasal congestion and aesthetic concerns such as a blunt nasal tip, low nasal bridge, and mild nasal contraction.

From the front, the nostrils were visible and the mouth appeared slightly protruded, making the face look unbalanced.

The patient’s main concerns were nasal congestion, a blunt nasal tip, and a low bridge.The skin at the nasal tip was thick, and the bridge appeared smaller than the tip, creating a droopy impression.

The nostrils were visible from the front, and the mouth projection emphasized facial imbalance.

Surgical Plan Using Self Rib Cartilage

To refine the blunt nasal tip, a portion of subcutaneous soft tissue beneath the tip skin was removed.The alar cartilage was rearranged and sutured to create a sharper, more defined contour.To elevate the nasal bridge, a silicone implant was used, and self rib cartilage (autologous rib graft) was applied at the nasal tip for strong structural support and lasting projection.

This surgical approach aimed to create a smooth, masculine, and natural nasal line with an improved nasolabial angle and a balanced facial profile.

Intraoperative Findings and Functional Correction

Preoperative CT scans and endoscopic findings revealed a rightward deviated septum and enlarged inferior turbinate.

During surgery, the septal cartilage was found to be thinner and more bent than expected.

The most curved sections of the septal cartilage were resected, and cross-hatching incisions were made to correct the concave deformities.

A high-frequency laser reduction was performed on the inferior turbinate to enhance nasal airflow and relieve congestion.

Postoperative Results and Review

This is a picture of the front right side. The drooping feeling has improved a lot, and the hanging columella has been slightly reduced, making the nasolabial angle more natural and the protruding mouth has improved. Considering that the swelling between the eyebrows will go away, the nose bridge line has also become more masculine, close to a straight line.
