September 2019: Revision resulting in nasal deformity (bulbous tip + drooping nose tip)
2020: Donated rib cartilage + silicone implant
2021: Ear cartilage graft
January 2024: Autologous rib cartilage + silicone implant

2. Patient’s Goals
Reduce excessive nostril visibility on the frontal view.
Elevate the nasal tip.
Enhance the height of the nasal starting point (bridge near the glabella).
Address insufficient volume on the left nasal dorsum.
Achieve a natural and subtle result with added nasal volume for a more refined look.
3. Preoperative Analysis and Findings
Through detailed examinations and precise analysis, the following structural and functional challenges were identified:
Weak nasal tip support due to multiple prior surgeries.
Significant asymmetry of the nostrils.
Volume deficiency on the left nasal dorsum.
Visible scars and loss of skin elasticity in the columellar and alar regions.
4. Surgical Approach

Reused existing autologous rib cartilage.
Enhanced stability using additional donated rib cartilage.
Strengthened the nasal tip support with a reinforced framework.

Addressed alar asymmetry caused by prior nostril reduction surgeries.
Adjusted the columellar height and achieved a balanced tip projection.
Corrected the nasal dorsum contour by filling depressed areas with cartilage and dermis.

Performed bilateral nostril corrections to restore symmetry.
Utilized composite skin grafting for areas with thinned or scarred skin.
Conducted columellar skin grafting to improve structural integrity and aesthetics.
