Today, I’d like to share the story of a patient who experienced severe nasal obstruction and asymmetry after a third rhinoplasty, which led to a narrowed nasal valve. In this case, we addressed the patient’s concerns by focusing on restoring functionality, improving nasal symmetry, and correcting the deviation.

3. Surgical Plan and Procedure
To address both structural and functional issues, we outlined the following surgical plan:
Lateral Osteotomy: Correcting nasal deviation with external osteotomy to straighten the bridge.
Septal Realignment and Extension: Using autologous rib cartilage to shift the septum slightly rightward and reconstruct stability.
Left Nostril Expansion and Scar Tissue Removal:
Excising scar tissue blocking the nasal passage
Performing a Z-plasty and skin graft from the patient’s ear to restore the nostril opening
Correction of Septal Deviation: Realigning the septum to improve airflow.
Turbinate Reduction: Using radiofrequency laser to reduce hypertrophy and improve nasal airflow.

4. Post-Surgical Results

The post-operative results successfully addressed both cosmetic and functional concerns:
Front View: The deviation was corrected, achieving a straighter nasal bridge and more symmetrical nostrils.
Side View: The nasal tip was slightly elevated, creating a refined, balanced profile with a natural, straight bridge.
Nostril View: The left nostril’s narrowing was significantly reduced, and the overall asymmetry was improved.

5.Final Thoughts
Reducing the size of the nostrils, if done excessively, can complicate future revisions, as was the case here. However, through meticulous scar removal, Z-plasty, and skin grafting, we successfully improved both form and function.
