Today, Iโ€™d like to share the case of a patient who previously underwent Hiko nose thread lift and filler augmentation, but returned after a year seeking a more natural, refined nasal lineย through corrective surgery. This was a case of bulbous tip refinement, thread removal, and subtle augmentation, all tailored to her aesthetic goals.

This case is a prime example of how even minor non-surgical procedures like Hiko threads and fillersย can sometimes result in long-term dissatisfactionโ€”especially when expectations arenโ€™t met or the aesthetic lacks harmony.


During consultation, I emphasized that overly conservative bulbous nose correction may leave patients underwhelmed, especially when subtle refinement doesnโ€™t create noticeable change. With this in mind, we pursued a moderate correctionย that preserved natural contours while delivering visible enhancement.


Using donor rib cartilageย and existing septal cartilage, I was able to rebuild and reshape the tip with precision. The patientโ€™s goal of a modest 1โ€“2 mm increaseย was achieved without compromising softness or femininity. Additionally, the lowered columella and improved nasolabial angle helped reduce the appearance of a protruding mouth.


From both a functional and aesthetic perspective, this surgery achieved long-lasting, balanced resultsโ€”without the use of silicone or aggressive reshaping.
