Hello, this is Dr. Cha-Young Kang, Director of Nose Lab Clinic.

Today, Iโ€™d like to share a rhinoplasty revision case where we transformed an overly artificial nasal bridge and severely upturned tip into a more natural, balanced profile.

Design Consultation: Frontal View, Left Side View, and Oblique Side View Photos

๐Ÿ“ Surgical History

  • 4โ€“5 years ago: Received nasal filler (later dissolved before surgery)

  • 4 years ago: Underwent rhinoplasty with silicone implant, septal extension, and donor rib cartilage graft

Design Consultation: Right Oblique Side View, Side View, and Nostril View Photos

๐Ÿ”Ž Initial Assessment

Frontal View

  • Overly upturned nose (short and over-rotated tip)

  • Excessive nostril exposure

  • Nostril asymmetry

  • Nasal implant appeared unnaturally rigidโ€”chalk-like in shape

  • High starting point of the bridge gave a harsh facial expression

Profile View

  • Extremely unnatural nasal dorsum curvature

  • Nostril exposure visible even in profile

Nasal Base

  • Deviated columella toward the right

  • Noticeable nostril asymmetry

โœจ Patientโ€™s Goals

  • Refine overly wide-looking nasal base

  • Reduce nostril exposure

  • Create a more harmonious nose that fits the face

  • Correct nasal deviation

๐Ÿ’‰ Surgical Plan & Techniques

Structural Correction

  • Septal extension using autologous rib cartilage

  • Downward rotation of the nasal tip to correct the upturned shape

  • Alar base repositioning to reduce nostril show

  • Osteotomy to correct deviated nasal bone

  • Columellar-labial angle refinement

Aesthetic Adjustments

  • Lowered the radix (starting point of the nasal bridge)

  • Softened and naturalized the dorsum curve

  • Overall reshaping to achieve a natural nasal contour

โœ… Surgical Outcomes

Revision rhinoplasty using autologous rib cartilage, with nasolabial angle correction and nostril extension.

Before (Left) / Immediately After (Right)

Frontal View

  • Tip rotation corrected for a more natural projection

  • Reduced nostril exposure

  • Improved nostril symmetry

  • Lowered radix created a softer expression

Before (Left) / Immediately After (Right)

Profile View

  • Smooth, natural nasal dorsum line

  • Minimal nostril visibility from the side

  • Harmonious silhouette with improved facial proportion

Before (Left) / Immediately After (Right)

Base View

  • Realigned columella

  • Symmetrical nostrils

  • Stable and balanced structural support

Before (Left) / Immediately After (Right)

This patient presented with a nose that appeared excessively artificial following a previous augmentation using silicone and donor rib cartilage. The most striking concerns were the chalky, overly rigid dorsum and severely upturned tip, both of which resulted in excessive nostril show and a harsh appearance.

To address these, we began by removing the unnatural implant and reconstructing the nasal framework using the patientโ€™s own rib cartilage. A key component was the septal extension graft, which allowed us to rotate the tip downward and reduce nostril visibility. Alar base repositioning further refined symmetry and narrowed the nasal width.

We also softened the nasal bridge by lowering the radix and reshaping the dorsum, transforming the previously harsh lines into a smooth, natural profile. The deviation of the columella and nostril asymmetry were corrected through precise graft placement and structural realignment.

This case exemplifies how even significantly artificial prior results can be reversed through careful planning and natural tissue reconstruction. Most importantly, we were able to deliver on the patientโ€™s request for a nose that felt in harmony with their overall appearance, restoring balance without creating an overly dramatic change.

Thank you,

Dr. Cha-Young Kang
