Today, Iโ€™d like to share a case of a patient who underwent revision rhinoplasty 17 years after initial dorsal hump correction. The patient presented with a recurring hump and significant nostril exposure that affected both aesthetics and facial harmony.

This patient presented a combination of aesthetic challenges stemming from a dorsal hump correction performed 17 years ago and a nasal filler procedure done 10 years ago. The primary concerns included a flattened and deviated nasal bridge, excessive nostril exposure from the front, and a drooping nasal tip with a large nasolabial angle from the side.


The priority in this revision surgery was to reduce the nostril exposure and establish a nasal height that harmonized with the rest of the facial features. We repositioned the nasal tip downward to conceal the nostrils more effectively and utilized autologous rib cartilage for septal extension, ensuring long-term structural stability.


Additionally, we performed lateral osteotomy to address the deviated nasal bridge and removed residual filler material to restore a clean anatomical foundation.


The result is a naturally refined nose that enhances facial balance. The adjustments brought symmetry to the nostrils and softened the noseโ€™s frontal and side profiles without creating an artificial look. The patientโ€™s request for a "harmonious and natural" enhancement was met through a meticulous, proportion-conscious approach.
