[Nose Lab Clinic]

Hello, this is Dr. Cha-Young Kang from Nose Lab Clinic.

Today, I would like to introduce a case of secondary rhinoplasty following the removal of a long-standing silicone implant. This patient experienced significant structural changes after implant removal and sought correction for nasal asymmetry and enhancement of a more feminine, refined tip shape.

Design Consultation: Frontal View, Left Side View, and Oblique Side View Photos

1. Surgical History

30 years ago: Silicone implant + auricular cartilage graft + alar base reduction

15 years ago: Secondary rhinoplasty (silicone implant replacement)

4 years ago: Silicone implant removal

2. Initial Assessment

a. Frontal View

✔ Bulbous nasal tip

✔ Mild nasal deviation

✔ Pinched alar region

✔ Widened nasal bridge after implant removal

This case was particularly complex due to multiple previous surgeries, including a 30-year-old silicone implant that was removed four years prior. The main challenges included:

✔ Addressing the widened and collapsed nasal bridge after implant removal

✔ Refining the nasal tip for a feminine and natural result

✔ Correcting nasal asymmetry while ensuring structural stability

Using a combination of rib cartilage grafting and a carefully selected silicone implant, we achieved both structural integrity and aesthetic enhancement. The final result was a naturally refined, elegant nasal shape that aligned with the patient’s preferences.

This is Dr. Cha-Young Kang, Chief Surgeon at Nose Lab Clinic.

Thank you.
