Preoperative Analysis and Surgical Plan in Closed Rhinoplasty Korea


This Closed Rhinoplasty Korea case involved a patient with a bulbous and wide nose tip, flat and curved dorsum, and mild deviation with retracted columella.

The goal of surgery was to achieve a natural, balanced nasal contour that harmonized with her facial proportions.

The nasal starting point was set between the double-eyelid crease and the pupil line, the nasal tip was slightly elevated, and the columella was lowered to adjust the nasolabial angle to approximately 90°, ensuring an elegant side profile.

In the preoperative side-view analysis, both the nasal bridge and tip were planned to be raised using septal cartilage and autologous rib cartilage, combined with a 4 mm silicone implant on the bridge for smooth dorsal elevation.

The frontal design emphasized correction of the bulbous tip, improvement of facial asymmetry, and straightening of the deviated dorsum through osteotomy.

To refine nasal width, bulbous nose correction and nasal floor reduction were also included in the surgical plan.

Addressing Deviation and Nostril Asymmetry


CT imaging taken before the operation revealed that the nasal septum was deviated to the left, which caused the left nostril to appear larger and the columella to tilt.

To resolve this, I repositioned the internal cartilage, narrowed the nasal base, and corrected the overall asymmetry through structural refinement inside the nose.

By repositioning the alar cartilages and adjusting the septal framework, the nasal tip could be stabilized and centered.

Postoperative photos show that the nasal tip became slimmer and more defined, while the columella descended naturally, creating a softer and smaller nasal impression.

The nasal bridge was elevated by about 4 mm, forming a natural and continuous dorsal contour.

At the 45-degree angle, the nasolabial angle improved noticeably, the lips appeared less protruded, and the previously blunt tip became refined and three-dimensional.

From the front view, the curved midline and the junction between the bridge and tip were much smoother, the nasal base narrower, and nostril asymmetry was significantly reduced.


A General Review and Surgical Insights


This Closed Rhinoplasty Korea case required precise handling of a bulbous, wide, and deviated nose with columella retraction, reconstructed with self-rib cartilage.

The septal cartilage was found to be more deviated than expected, so it was reinforced with donated rib cartilage for stability.

Patients undergoing this type of rib cartilage reconstruction rhinoplasty are advised to avoid trauma for at least six months after surgery to ensure stable healing and maintain symmetry.

Overall, the surgery achieved a natural and balanced improvement with enhanced nasal definition and harmony.
