This patient presented with a complex case involving nasal tip transparency caused by a previously placed artificial dermal graft and a desire to visually shorten the midface. The patient’s known allergies to psoriasis and hyaluronidase required extra caution in surgical planning and material selection.

The most pressing concern was the visible transparency at the nasal tip. Artificial dermal grafts can initially provide volume but often lead to contour irregularities or transparency issues over time as they interact with surrounding tissues. Complete removal was essential to prevent further complications.

We reconstructed the nasal framework entirely using autologous rib cartilage to ensure long-term safety and aesthetic stability, while providing adequate tissue thickness to prevent future transparency.

To address the patient’s request for midface proportion correction, we adjusted the columella angle and elevated the nasal tip to create the visual effect of a shortened midface and an overall balanced facial profile. The glamorous nasal line the patient desired was achieved without compromising naturalness.
