Hello, this is Dr. Cha-Young Kang, Director of Nose Lab Clinic.


Today, I’m sharing a case involving a patient who struggled with a highly visible nostril line (upturned nose) and a wide nasal base, seeking a natural yet defined nasal contour without excessive length or artificial appearance.

🔎 Initial Consultation


1. Frontal View

  • Excessive nostril exposure (upturned appearance)

  • Wide nasal bridge

  • Low nasal dorsum

  • Slightly crooked nasal axis


2. Side Profile

  • Nostril exposure evident from the side

  • Protruding upper lip

  • Irregular dorsal contour


3. Nasal Base

  • Enlarged nostrils

  • Rightward deviation of the columella

  • Mild nostril asymmetry


Patient Goals

  • A natural-looking nose with a well-defined profile

  • Reduce nostril exposure

  • Correct upturned tip

  • Avoid an overly long or artificial result


💉 Surgical Approach


1. Structural Correction

  • Repositioned the nasal tip to correct the upturned shape

  • Repositioned alar cartilages to reduce nostril exposure

  • Refined the broad nasal bridge


2. Profile Refinement

  • Smoothed the irregular nasal dorsum

  • Sculpted a natural, defined profile

  • Maintained an appropriate nasal length


3. Functional Treatment

  • Submucosal radiofrequency turbinate reduction for chronic rhinitis

  • Septoplasty for deviated septum

This case was focused on achieving a delicate balance: correcting the upturned tip and nostril exposure while avoiding an overly long or artificial look. The patient wanted a result that was defined yet natural, and we tailored the surgical plan accordingly.


Functional procedures were also essential, addressing nasal obstruction due to turbinate hypertrophy and a deviated septum. These functional aspects are eligible for medical insurance coverage in many cases.


Through precise cartilage repositioning, dorsal smoothing, and tip refinement, we achieved a nose that feels natural in both structure and aesthetics, fulfilling the patient’s goals of having a line that is alive, but never overdone.
