Hello, this is Dr. Cha-Young Kang from Nose Lab Clinic.


Today, I’ll share a case of rhinoplasty for a patient who underwent orthognathic (jaw) surgery 10 years ago. The goal was to create a natural-looking hump nose (dorsal hump preservation) while correcting nasal deviation using autologous rib cartilage.

3. Patient’s Goals

4. Surgical Plan & Execution


a. Structural Reconstruction

✔ Rebuilt the weakened septal cartilage (damaged from orthognathic surgery) using autologous rib cartilage

✔ Performed lateral osteotomy (bone reshaping) to straighten the deviated nose

✔ Extended the septum with rib cartilage to lengthen the nasal tip


b. Hump Nose Refinement

✔ Sculpted the rib cartilage to preserve a natural-looking dorsal hump

✔ Customized bridge height based on patient preference

✔ Ensured a smooth, harmonious nasal contour


c. Detail Adjustments

✔ Corrected nasolabial angle (columella-labial angle) for a proportional appearance

✔ Lowered the columella to balance the facial profile

✔ Refined and slimmed the bulbous nasal tip

This case was a successful transformation of a post-orthognathic surgery nose into a natural-looking dorsal hump nose, a style that is gaining popularity.


1️⃣ The patient's septal cartilage was significantly weakened from previous orthognathic surgery, requiring rib cartilage reconstruction to provide long-term stability.


2️⃣ The patient desired a refined but not completely straightened bridge, preferring to maintain a subtle hump for a more natural and distinctive look.


3️⃣ By adjusting the columella position, the final outcome not only improved nasal aesthetics but also enhanced overall facial balance.


🔸 Autologous rib cartilage was used for durability and safety, avoiding synthetic implants while ensuring a natural and stable result.
