Refining Nostril Asymmetry and Drooping Tip โ€“ A Subtle and Natural Revision Rhinoplasty Case

Hello, this is Dr. Cha-Young Kang, Director of NOSELAB Clinic.


Today, Iโ€™d like to share a case of revision rhinoplasty for a patient who struggled with nostril asymmetry and a drooping nasal tip, while hoping to preserve her natural appearance.

  • 6 years ago: Silicone removal surgery

  • 1 year ago: Filler injection (not dissolved)

  • Congenital nostril asymmetry

Structural Reconstruction

  • Septal extension using donated rib cartilage and the patientโ€™s own septal cartilage

  • Combination approach chosen to reduce potential resorption and shape distortion from donated cartilage

  • Complete removal of residual filler and foreign material

  • Nasolabial angle correction for better profile balance

Aesthetic Refinement

  • Correction of nostril asymmetry within the limits of congenital alar base differences

This case involved a combination of structural and aesthetic concerns, including previous non-surgical procedures and congenital nasal asymmetry. The patientโ€™s primary concern was the visibly uneven nostrils, which were partly congenital and therefore not completely correctableโ€”but we focused on achieving the most natural-looking improvement possible.


Given her smaller nose structure, we carefully avoided over-elevating the tip while still addressing the droop effectively. Although the patient preferred not to use her own rib cartilage, we created a stable foundation by combining donated rib cartilage with her septal cartilage. This hybrid approach minimized the risk of warping or resorption typically associated with donated cartilage alone.


We also removed all residual filler and traces of previous silicone, and refined the nasolabial angle to improve side profile harmony. Importantly, we ensured her overall appearance remained familiarโ€”honoring her request to preserve her current image while subtly correcting problem areas.


This case illustrates how tailored revision surgery, even within structural limitations like congenital nostril asymmetry, can deliver a naturally beautiful and balanced result.
