Hello, I’m Dr. Cha-Young Kang, Director of Nose Lab Clinic.

Today, I’ll discuss a complex case of severe contracted nose revision resulting from multiple previous rhinoplasties. This case illustrates the importance of meticulous planning and the use of autologous rib cartilage for successful reconstructive outcomes.

1. Patient Information and Symptoms in Severe Contracted Nose Revision

Surgical History:

  • 1st Surgery: Silicone implant + Ear cartilage graft + Nostril reduction

  • 2nd Surgery: Autologous rib cartilage revision

  • 3rd Surgery: Additional A/S procedure

Primary Symptoms:

  • Upturned, visible nostrils

  • Excessively elevated nasal tip

  • Scar tissue accumulation

  • Breathing difficulties due to obstruction

2. Pre-Surgical Analysis

Front View: Overly elevated nasal tip with nostril asymmetry

Side View: Sharp, upturned nasal tip creating a shortened, unnatural appearance

Nostril View: Columella deviated to the left with visible scarring

Functional Issues: Deviated septum and external nasal valve collapse

3. Surgical Plan for Severe Contracted Nose Revision

We aimed to restore both function and aesthetics through structural reconstruction.

Primary Procedures:

  • Septal Reconstruction with Autologous Rib Cartilage: Strengthened the septum to support and reposition the tip for a natural orientation.

  • Granulation Tissue Removal: Eliminated inflamed tissue to reduce contracture risk.

  • Nostril Lowering and Symmetry: Balanced nostril appearance by lowering the base.

  • Columella Adjustment: Softened projection for a natural look.

  • Alar Cartilage Reconstruction: Rebuilt and repositioned for symmetry.

  • External Nasal Valve Reconstruction: Widened the airway to improve breathing.

4. Post-Surgery Outcome

  • Nasal Tip and Nostril Positioning: Natural tip orientation with improved nasolabial angle.

  • Nostril Shape: Corrected asymmetry and rounded the nostril rims.

  • Structural Reinforcement: Restored breathing by widening the nasal valve area.

Conclusion

This severe contracted nose revision case highlights the need for a staged and systematic approach. At Nose Lab Clinic, we combine extensive experience with advanced methods like autologous rib cartilage reconstruction to achieve safe and lasting outcomes.

For patients considering revision rhinoplasty, consulting an experienced surgeon is essential.
