Hello, this is Dr. Cha-Young Kang, the head surgeon at Nose Lab Clinic.
Today, I’m excited to share a rhinoplasty case where we successfully corrected a nose that appeared too long for the patient’s mid-face area, giving it a more elegant and balanced look.
This patient had previously undergone multiple surgeries and was seeking a more harmonious overall facial proportion through rhinoplasty.
1. Patient's Surgery History
2016: Nose filler injection
2020: Silicone + donor rib cartilage + nostril reduction
2021: Philtrum reduction surgery
2. Patient's Concerns and Requests
Correction of a crooked nose.
Improvement of sagging nostrils
Reducing the appearance of a long mid-face
Creation of a straight nasal line
Enhancement of a narrow-looking nasal bridge
Correction of a pinched nostril appearance

Lowering of the nasal starting point (glabella)
Smoothing out the sharp appearance of the nostrils
Correction of nostril asymmetry
3. Pre-Surgery Design Consultation
We analyzed the patient’s pre-surgery condition through photos:

Frontal View:
The nose appears crooked.
The silicone implant is visible through the thin skin.
The nostrils look pinched.
The columella looks sunken.
Oblique View:
The nasal tip appears slightly droopy.
Side View:
The nostrils seem saggy.
The nasal starting point is too high.
The nasal tip is slightly droopy.
The columella is sunken, and the nasolabial angle is less than 90°.

The columella appears tilted to the right.

4. Surgery Plan
Based on the patient's condition and requests, we created the following surgery plan:
a) Reconstruction of the septal cartilage using autologous rib cartilage
b) Septal extension surgery to adjust and raise the nasal tip
c) Lowering of the columella and correction of the nasolabial angle
d) Repositioning and reshaping of the alar cartilage to correct nostril asymmetry
e) Replacing the silicone implant with a broader one for better proportions
f) Lowering the nasal starting poin g) Nostril correction to improve the overall shape
5. Surgery Details
We performed the surgery using a closed (endonasal) approach, completing the following:
a) Autologous rib cartilage harvest and septal cartilage reconstruction:
The previously damaged septal cartilage was reconstructed using the patient’s rib cartilage.
b) Septal extension surgery:
The nasal tip was raised to give a more refined appearance.
c) Lowering of the columella and nasolabial angle correction:
The columella was gently lowered, adjusting the nasolabial angle to around 95°.
d) Repositioning and reshaping of the alar cartilage:
The nostrils were reshaped, and asymmetry was corrected.
e) Replacing the silicone implant:
The previous implant was replaced with a broader one to reduce the visibility of the implant under the skin.
The nasal starting point was lowered to improve the overall facial proportions.
f) Nostril correction:
The sharp appearance of the nostrils was softened, and symmetry was restored.

8. Conclusion
This case is a great example of how rhinoplasty can completely transform the face, creating a more balanced and harmonious look. By lowering the nasal starting point and correcting the nasolabial angle, we were able to reduce the impression of a long mid-face and enhance the overall facial proportions.
The length and shape of the nose can significantly influence the overall facial appearance. When the mid-face is long or the face appears elongated, a high nasal starting point can exaggerate this effect. In such cases, lowering the nasal starting point and adjusting the nasolabial angle can create a shorter-looking nose and improve the balance of the entire face.
In this case, lowering the nasal starting point and adjusting the nasolabial angle to 95° helped reduce the perceived length of the mid-face and improve the overall facial proportions. Additionally, we addressed multiple issues such as a crooked nose, narrow-looking bridge, and nostril asymmetry, creating a harmonious and natural nose.
9. Final Thoughts
The length of the nose plays a significant role in the overall facial balance and impression. For those with a long mid-face or elongated facial appearance, adjusting the nasal starting point can dramatically improve facial harmony.
By making precise adjustments to the nasal starting point and nasolabial angle, we can create a more balanced, harmonious look. This type of surgery is not just about changing the shape of the nose but improving the overall balance and symmetry of the entire face.
