Hello, this is Dr. Chayoung Kang, Director of Noselab Rhinoplasty Clinic. Today we will introduce the case of a patient who was concerned about her wide nose. This patient said she wanted to improve the width of her nose, and decided to get a bulbous nose rhinoplasty.
Preoperative Design Consultation

Surgical content
The surgery is performed entirely through closed rhinoplasty approach, as follows:
a) Bulbous nose correction:
Partial removal of soft tissue: reduction of the thickness of the skin by partially removing subcutaneous soft tissue from both lateral sides of the nose tip.
Tying the cartilage: improving the appearance of the nose by relocating and tying up the alar cartilage.
Nasal tip plasty: giving a more three-dimensional appearance to the nose tip by increasing its height.
b) Outer osteotomy of the nasal bone:
Performing an outer osteotomy to correct the wide nasal bone.
c) Nasolabial angle correction surgery:
Correction of the nasolabial angle to give an improvement effect to the protruding mouth.
d) Nasal valve reconstruction:
Performing a nasal valve reconstruction after it was suspected through a nasal congestion examination the possibility of medial valvular stenosis.
Additional surgical contents:
Reinforcement of the thin and small alar cartilage using the septal cartilage.
Every procedure was performed through the closed rhinoplasty approach, without leaving any external scars.

Overview
This case is a successful example of a patient with a wide nose, also known as a bulbous nose. Bulbous nose rhinoplasty is not simply about reducing the size of the nose, but it is also important to create a natural and beautiful nose by considering the balance of the entire face. In this surgery, a combination of techniques such as partial removal of subcutaneous tissue, cartilage tying, and nose tip surgery was used to correct the wide nose. In particular, when the cartilage of the alar alar is thin and small, it is difficult to achieve good results by simply reducing its size. Therefore, by reinforcing the cartilage using the septum cartilage, we were able to secure structural stability of the nose while also creating a beautiful shape.
Correcting a wide nasal bone through outer nasal bone osteotomy and improving the harmony between the nose and lips through nasolabial angle correction also played a major role in improving the overall balance of the face.
In addition, in this case, functional improvement was achieved along with cosmetic improvement. By performing nasal valve reconstruction surgery to correct the medial nasal valve stenosis discovered in the nasal congestion examination, the patient’s respiratory function also improved.
All procedures were performed through the closed rhinoplasty method, which minimizes scarring and ensures a short recovery period.
