Bulbous and Blunt Nose Surgery_Rhinoplasty Korea

Hello, this is Dr. Chayoung Kang, Director of Noselab Rhinoplasty Clinic. Today we will introduce the case of a patient who was concerned with a bulbous and blunt nose.


This patient had gotten nose filler 12 years ago, and after getting nose reduction injections, she said she developed symptoms of a hole appearing on the skin of the nose. With this surgery, she wanted to make her blunt nose into a sophisticated nose, and wanted to make her narsal dorsum straight without any curve.

Surgical Details


The following surgeries were performed through closed rhinoplasty:


a) Lateral osteotomy:


  • A lateral osteotomy to narrow the wide nasal bone.

  • This reduced the overall width of the nose and created a slimmer line.


b) Septal extension graft:


  • Since the patient’s septal cartilage was not large, we used donated rib cartilage.

  • This makes the tip of the nose straight and increases support.


c) Nose bridge heightening:


  • Usage of silicone implants to increase the height of the nose bridge.

  • We created a straight nose bridge without any curves as the patient desired.


d) Correction of nasolabial angle:


  • The nasolabial angle was appropriately adjusted to improve the protrusion of the mouth.


e) Remove existing filler:


  • Removal of the remaining filler on the bridge of the nose to improve the uneven appearance.


f) Correction of nasal tip asymmetry


  • The symmetry was improved by straightening the tip of the nose and the bridge that were tilted to the right.


All procedures were performed using a closed approach, completing the surgery without external scars.

  • The nasal hump appearance improved and changed into a smooth line.

  • As we solved the blunt nose tip issue, we could observe a more sophisticated appearance.

Overview – Bulbous nose surgery


This case is a successful case of a bulbous nose that had complex problems due to multiple procedures. It is particularly noteworthy that we resolved the side effects (sunken nose tip, bumpy nose bridge) from previous filler and sculpting injection procedures resolved while achieving the patient’s desired appearance.

We coprrected the wide nasal bone through a lateral osteotomy, and we performed a septal extension graft to correct and to give a three-dimensional appearance, which improved the overall look of a bulbous nose. In particular, the use of donated costal cartilage was an important choice for stable and long-term results, as the patient did not have enough septal cartilage.

Removing the foreign substances remaining on the bridge of the nose and raising the bridge of the nose using a silicone implant was effective in creating the straight nose that the patient wanted. In addition, by correcting the angle of the nasolabial fold, we were able to improved the nasolabial angle, thereby improving the overall balance of the face.

By using a closed approach, we were able to shorten the postoperative recovery period and complete the surgery without leaving a scar, which greatly contributed to increasing patient satisfaction.
