Hello, Iโ€™m Dr. Cha-Young Kang, Medical Director at NOSELAB Clinic.

Today, Iโ€™d like to share the story of a patient who underwent rhinoplasty to correct a bulbous nose and remove residual substances from previous filler and thread lift procedures, resulting in a balanced and naturally contoured nose.

๐Ÿ“ Procedure History

  • 2020: Nasal filler injection

  • 2023: Thread lift procedure (nasal threads)

๐Ÿ”Ž Initial Assessment

Key Features

  • Bulbous nasal tip (typical โ€œbulbous noseโ€ appearance)

  • Suspected residual filler and thread materials

  • Irregular nasal contour

  • Drooping nasal tip

  • Nostril asymmetry

โœจ Patient Goals

  • Correction of bulbous tip

  • Natural yet defined nasal contour

  • Removal of prior filler/thread materials

  • Accepts a mild hump (no need for a fully straight profile)

๐Ÿ’‰ Surgical Plan

Consultation Highlights

  • Patient initially preferred implant-free surgery

  • Explained that subtle correction may not be sufficient for effective bulbous tip refinement

  • After thorough discussion, the patient agreed to silicone implant use for optimal structural enhancement

Structural Reconstruction

  • Complete removal of residual filler and threads

  • Cartilage grafts using septal and donated rib cartilage

  • Silicone implant placed for improved dorsal definition

  • Functional surgery for nasal obstruction (septal deviation and turbinate hypertrophy)

Aesthetic Refinement

  • Tip refinement for better definition

  • Correction of nasal tip ptosis

  • Improved nostril symmetry

  • Harmonious height and overall contour

โœ… Postoperative Outcomes

Frontal View

  • Markedly improved tip definition

  • Balanced and symmetrical nostrils

  • Overall more proportionate and refined nose

Before (Left) / Immediately After (Right)

Side View

  • Naturally elevated nasal bridge

  • Corrected nasal tip droop

  • Ideal height without appearing overdone

  • Preserved a soft, slightly curved profile

Before (Left) / Immediately After (Right)

Before (Left) / Immediately After (Right)

Nostril View

  • Improved nostril symmetry

  • Stable nasal base and support structure

  • Natural nostril shape and angle

This case involved a patient who had received nasal fillers in 2020 and thread lifts in 2023, resulting in a bulbous and uneven nasal shape. Initially, the patient preferred an implant-free approach, but I explained that in cases involving a naturally low and flat nasal structure, particularly with a bulbous tip, subtle corrections often fail to produce meaningful change.

Through detailed consultation, we agreed that a silicone implant would offer better projection and shape, particularly important for defining the tip and bridge in bulbous noses.

The first priority was the complete removal of foreign materials from prior procedures. We then reconstructed the nasal framework using both septal and donated rib cartilage, supported by a silicone implant. This dual approach allowed us to build a strong, stable base while also refining the nasal line.

The surgery also addressed functional issues with breathing by correcting a deviated septum and reducing turbinate hypertrophy, which are often overlooked but crucial for long-term satisfaction.

By correcting the droopy tip and nostril asymmetry, we achieved a harmonious result that aligned with the patientโ€™s preferencesโ€”avoiding an overly straight profile while improving overall balance and definition.

Thank you for reading.
