Bulbous Nose Correction with Upturned Nose – Revision Rhinoplasty Korea

Hello, this is Dr. Cha-Young Kang, Director of Nose Lab Clinic.


Today, I’d like to introduce a rhinoplasty case for a patient who was concerned about a bulbous and upturned nose. The patient desired a natural nasal shape, and we planned a customized surgery balancing safety and aesthetics.


Pre-Surgery Design Consultation for Bulbous Nose Correction

Patient's Requests:
  • A defined and slightly elevated nasal tip

  • A straight nasal profile (creating a natural “straight nose”)

  • A rounded, but not overly sharp nasal tip

  • Natural results without obvious signs of surgery

  • Use of rib cartilage if possible (though the patient preferred septal cartilage)

Front View:
  • Nostrils highly visible → upturned nose appearance

  • Nasal tip bulbous and elevated

  • Low nasal root, creating a flat profile


Side View:
  • Nose appeared short and upturned

  • Flat bridge with lack of definition


Oblique View:
  • Short nose appearance

  • Subtle dorsal hump visible


Nasal Base:
  • Slight nostril asymmetry

  • Broad nasal tip lacking definition

Surgical Procedure – Closed Rhinoplasty for Bulbous and Upturned Nose


  • Septal Cartilage Harvesting:A limited amount of septal cartilage was taken with some bone for stability. Septal extension graft was performed to support the structure.


  • Nasal Tip Correction:Septal cartilage was used to lower the upturned tip and refine the bulbous shape. Ear cartilage was avoided to reduce invasiveness.


  • Dorsal Hump Correction and Augmentation:Irregularities were smoothed. A silicone implant was inserted to enhance the bridge, creating a more refined and balanced profile.


  • Nostril and Nasal Shape Refinement:The bulbous nasal tip was slimmed, asymmetry of nostrils corrected, and the dorsal line refined into a smooth contour.


All procedures were performed using a closed (endonasal) approach, leaving no external scars.

  • Upturned tip corrected, facial balance improved

  • Short appearance corrected with better proportion

  • Smooth line from glabella to tip created

  • Improved nasal length and balance

  • Subtle hump corrected for smooth contour

  • Natural, straight dorsal line achieved

Final Evaluation

This case demonstrates how bulbous nose correction and upturned tip correction can be achieved with careful planning and closed rhinoplasty. By respecting the patient’s preference to avoid rib cartilage, we maximized results using septal cartilage while ensuring safety.
