Hello, this is Dr. Chayoung Kang, Director of Noselab Rhinoplasty Clinic. Today, I would like to introduce a corrective surgery case of a patient who presented with a bulbous nose, along with deviation to the right side and an upturned nasal tip.

The patient’s main concerns included the bulbous nose correction of her blunt and wide tip, improvement of the deviated and upturned appearance, and the desire to increase the height of the nasal starting point.

Surgical details

a) Autologous rib cartilage harvesting and transplant

Harvestingf the costal cartilage to secure a sufficient amount of cartilage needed for nasal reconstruction.

b) Septal extension surgery:

Strenghtening and extension of the septal cartilage using autologous costal cartilage.
This naturally lowers the tip of the nose and increases support.

c) Nose tip correction:

The blunt nose tip was transformed into a natural shape by removing the subcutaneous tissue, repositioning the alar cartilage, and tying the cartilage.
The height of the tip of the nose has been appropriately adjusted to improve the feeling of a bulbous nose.

d) Deviated septum correction:

The right-sided deviation of the septum was corrected to improve the tip of the nose, and the overall symmetry of the nose was improved.
Through this, nostril asymmetry was also naturally corrected.

e) Lowering the starting point :

The starting point of the nose bridge, which appeared high, was appropriately lowered to create a natural connection with the glabella.

f) Nasolabial angle correction:

Slight lowering of the columella to adjust the nasolabial angle appropriately.
Throught this, we were able to improve the artificial-looking upturned nose and transform it into a more natural-looking nose.

Every procedure was performed through the closed rhinoplasty approach, without leaving any external scars.

Overview – Bulbous Nose Correction

This case is one that simultaneously solved the complex problems of a bulbous nose, a crooked nose, and a deviated nose. The patient’s nose shape before surgery was not severe, but it felt unnatural and artificial overall. Septal extension surgery using autologous costal cartilage played a major role in correcting the upturned nose tip and strengthening the overall structure of the nose. In addition, by straightening the deviated septum, we were able to solve the problem of nostril naturally. Lowering the starting point of the bridge of the nose and appropriately adjusting the nasolabial angle played a significant role in solving the problem of the nose looking artificial. This allowed us to create an overall natural and harmonious nose shape.

By using a closed rhinoplasty approach, we were able to shorten the postoperative recovery period and complete the surgery without leaving a scar, which greatly contributed to increasing patient satisfaction.
