This Closed Rhinoplasty Korea case features a patient who visited NoseLab seeking to refine a bulbous nasal tip and wide alar base. The tip of the nose appeared blunt and droopy, giving an unnatural appearance. A slight dorsal hump and low nasion further emphasized the bulbous shape of the nasal tip.

During examination, both alars were large, and the columella was slightly retracted.Rather than reducing the nasal alar base, which often causes visible scarring or asymmetry, I designed a surgical plan to achieve natural refinement by lowering the columella and adjusting the nasolabial angle to approximately 90 degrees.The lower nasal width was reduced slightly to address the overall spread, and autologous rib cartilage was used to raise the nasal tip, providing a more three-dimensional, natural look.

This was a surgery to correct a nose that looked overly large and wide.Many patients assume that alar base reduction is necessary, but this procedure carries a high risk of permanent scars, asymmetry, and unnatural narrowing of the nostrils.Instead, by lowering the columella and refining the nasal tip through a closed approach, a natural and proportional appearance was achieved — without external cuts or irreversible changes.
