This case involves a patient who visited our clinic with concerns about a deviated nose and a prominent columella that made her nose appear overly long, especially in profile. The drooping appearance of the columella disrupted overall facial balance. To correct both issues without external scarring, we planned a closed rhinoplasty Korea approach focused on structural refinement rather than simple reduction.


Surgical history:

  • 5 years ago: Nasal filler injection (dissolved before surgery)

  • 2 years ago: Silicone implant rhinoplasty with ear cartilage graft

Surgical Plan – Closed Rhinoplasty Korea Using Rib Cartilage

A closed rhinoplasty technique was selected to preserve nasal soft tissue and avoid visible scars. Autologous rib cartilage was used to reconstruct and reposition the nasal tip, allowing controlled adjustment of columella length and nasolabial angle.

  • Removal of previously grafted ear cartilage

  • Tip reconstruction using autologous rib cartilage for stable support

  • Repositioning of tip structures to reduce columella show

A long or drooping columella should not be corrected by simple trimming alone. In this case, the closed rhinoplasty Korea approach allowed precise repositioning of the nasal tip without external incisions. Using autologous rib cartilage provided strong structural support, ensuring long-term stability while maintaining a natural appearance.
