Comprehensive Correction of Upturned Nose, Hump Nose, and Crooked Nose_Rhinoplasty Korea

Hello, this is Dr. Cha-Young Kang, head surgeon at Nose Lab Clinic.


Today, I would like to introduce a rhinoplasty case involving a patient with multiple nasal concerns, including an upturned nose, hump nose, and crooked nose. This successful surgery demonstrates how we can address several nasal issues simultaneously, and I will explain the methods and results in detail.


1. Pre-Surgery Design Consultation


A detailed analysis of the patient’s pre-surgery condition revealed the following:

Frontal View:

  • The nose appeared crooked.

  • The nasal tip was bulbous and wide.

  • The nostrils were overly visible, characteristic of an upturned nose.

  • The philtrum appeared long.


Oblique View:

  • The droopy nasal tip was even more pronounced.

  • Both the hump and upturned characteristics of the nose were noticeable.


Side View:

  • The nasal tip appeared droopy.

  • A hump nose was clearly visible.

  • The nasal tip appeared short.


Nostril View:

  • Nostril asymmetry was prominent.

  • The nasal base appeared wide.

Patient’s Key Requests:


  1. Reduce visibility of the nostrils from the front.

  2. Smooth the hump on the bridge.

  3. Improve the overall balance and harmony of the nose.

2. Surgical Plan and Procedure


We performed the surgery using a closed (endonasal) approach without creating any external scars, following this plan:


a) Septal Extension Graft:

  • Using septal cartilage and donor rib cartilage, we extended and refined the nasal tip to create a more elevated and refined appearance, addressing both the droopy and upturned characteristics of the nose.


b) Nasal Tip Refinement:

  • The wide, bulbous tip was reshaped using cartilage trimming techniques, giving the tip a more delicate and refined form.


c) Lateral Osteotomy:

  • The crooked nasal bridge was corrected, and the wide nasal base was narrowed using lateral osteotomy.


d) Nasolabial Angle Correction:

  • By adjusting the position of the nasal tip in relation to the upper lip, the long philtrum was visually shortened, improving the facial balance. This also helped reduce the appearance of a protruding upper lip.


e) Dorsal Augmentation:

  • We used a silicone implant to elevate the nasal bridge, ensuring the hump was corrected and creating a smooth, straight profile.


f) Nostril Asymmetry Correction:

  • Cartilage repositioning and suturing techniques were employed to improve the symmetry of the nostrils.


3. Before and After Photo Comparison

Frontal View:

  • The crooked nose has been corrected, and the nose now appears straight.

  • The wide, bulbous nasal tip has been refined into a sleeker, more defined shape.

  • The nostrils, previously too visible, are now naturally concealed.

  • The philtrum appears shorter, enhancing the overall facial balance.

Side View:

  • The droopy nasal tip has been elevated, and the nose now forms a smooth, straight line.

  • The hump nose has been corrected, resulting in a clean and elegant profile.

  • The overall length and height of the nose have been improved, enhancing facial harmony.

  • The droopy nasal tip has been corrected, resulting in a more three-dimensional and harmonious nose shape.

  • The simultaneous correction of both the hump nose and upturned nose has resulted in a natural, balanced nasal contour.

Nostril View:

  • The previously asymmetrical nostrils are now even and symmetrical.

  • The wide nasal base has been narrowed, contributing to a more refined appearance.


4. Final Evaluation


This case demonstrates the successful correction of multiple nasal issues, including an upturned nose, hump nose, and crooked nose. The key elements of this surgery included:


  1. Septal Extension Graft:


  • By utilizing both septal and donor rib cartilage, the septal extension graft played a crucial role in correcting both the droopy and upturned nasal tip, providing stability and a natural shape.


  1. Lateral Osteotomy:


  • This technique was essential in correcting the crooked nasal bridge and achieving facial symmetry.


  1. Nasolabial Angle Correction:


  • By adjusting the nasolabial angle, we were able to visually shorten the philtrum, improving the balance between the nose and lips, which also enhanced the overall appearance of the face.


  1. Nostril Asymmetry Correction:


  • The repositioning and suturing of the cartilage improved the symmetry and natural appearance of the nostrils, contributing to a balanced and harmonious result.


The main goal of this surgery was to achieve a natural and harmonious appearance while addressing both a long nose (hump nose) and a short nose (upturned tip). When a nasal hump is present with a short nasal tip, careful planning is necessary to balance the two conflicting features.


If the balance is not achieved correctly, the nose can either appear too long or too short, depending on how the tip and hump are adjusted. In this case, we achieved a natural correction by extending the nasal tip while refining the hump, using donor cartilage and silicone to create a balanced, harmonious outcome.


5. Closing Remarks
