Correcting Upturned and Crooked Nose: A Natural Transformation_Rhinoplasty Korea

Hello, this is Dr. Cha-Young Kang, head surgeon at Nose Lab Clinic.


Today, I’d like to introduce a case where we successfully corrected an upturned and crooked nose while achieving a more natural and balanced look. The patient had previously undergone two rhinoplasty surgeries: the first using an L-shaped silicone implant and the second using both silicone and ear cartilage. The patient was unsatisfied with the unnatural appearance and sought a more refined and harmonious result.


1. Pre-Surgery Consultation and Analysis


Patient Requests:

  • Correct the artificial look.

  • Prefer a straight (rectilinear) nasal profile.

  • Lower the nasal starting point.

  • Bring the nasal columella (the tissue between the nostrils) downward.

  • Maintain the current nose length without making it longer.

Frontal View:

  • The nasal bridge appears crooked.

  • The nostrils are overly visible and asymmetrical.


Oblique View:

  • The nose has an overly upturned appearance with an unnatural, ski-slope-like curve (concave shape).


Side View:

  • The nasal starting point is too high.

  • The nasal tip is overly elevated.

  • The lips appear slightly protruded.

Nostril View:

  • The nasal tip is tilted to the right.

  • Nostril asymmetry is evident.


2. Surgical Plan


Based on the patient’s condition and requests, we established the following surgical plan:


1) Septal extension graft using autologous rib cartilage:

  • Correct the upturned nasal tip.

  • Adjust nostril asymmetry.


2) Replace the silicone implant:

  • Remove the old implant.

  • Insert a new, shorter and wider implant to lower the starting point of the nasal bridge.


3) Columella lowering and nasolabial angle correction:

  • Improve the long philtrum (upper lip length).

  • Enhance the overall facial profile, particularly addressing the protruding lips.


4) Functional Improvements:

  • Correct deviated septum.

  • Perform radiofrequency-assisted turbinate reduction to treat hypertrophic rhinitis.


3. Surgical Procedures


Using a closed rhinoplasty approach, we performed the following:


1) Harvesting autologous rib cartilage and septal extension graft

2) Replacement of the silicone implant

3) Columella lowering and nasolabial angle correction

4) Septoplasty

5) Radiofrequency-assisted turbinate reduction


4. Before and After Comparison

Frontal View:

  • The previously crooked nasal bridge has been straightened.

  • The nasal tip, which was overly upturned, is now naturally lowered.

  • Nostril exposure and asymmetry have been improved.

  • The unnatural, overly concave (ski-slope) shape has been replaced by a more natural straight (rectilinear) line.

  • The nasal bridge and tip now appear softer and more harmonious.

  • The nasal tip is now in a more natural position, with the columella lowered to create a more balanced profile.

  • The nasolabial angle has been corrected, reducing the appearance of protruding lips.

  • Significant improvement in nostril asymmetry and shape has been achieved.

6. Conclusion


This case is a great example of how we corrected an overly artificial-looking nose to achieve a more natural, balanced result. Specifically, the ski-slope-like nasal profile was transformed into a more rectilinear shape, and the overly upturned nasal tip was brought down to a natural position. The overall facial proportions were improved by lowering the nasal bridge starting point and adjusting the columella.


Importantly, the surgery not only achieved aesthetic improvements but also addressed functional issues such as nasal obstruction caused by a deviated septum and hypertrophic rhinitis. This demonstrates the benefit of combining both cosmetic and functional expertise in rhinoplasty.


7. Final Thoughts
