There is a lot of curiosity surrounding the methods, and types of materials that can be used in rhinoplasty. Since the nose is at the center of the face, and changing it determines the ratio and overall appearance of the face, most people hope to get perfect results.
However, it is important to have a clear understanding of what is necessary in order to achieve a nose that harmonizes with one’s face, rather than solely deciding on the desired nose shape before surgery. To explain further, today I would like to present the real case of a patient who underwent surgery for a curved and bulbous nose.
Before surgery, the patient’s nose had these characteristics
This patient had a “droopy nose tip, and a wide bulbous nose. The nose bridge was slightly hooked, and deviated. This was a bulbous and deviated nose corrective surgery.” In person, the most characteristic parts were seen from the frontal view, the nose looked wide, and the area between the eyebrows had a plan and flat appearance. The base of the nose also was on the wider side.
The surgery was performed this way.
To improve these parts, I present the surgery plan below as it was explained to the patient.
To fix the bulbous nose, we need to reduce the skin thickness, and after relocating the cartilage, we will heighten the nose tip altogether by giving it a three-dimensional effect.
We would push the droopy columella slightly inwards, and correct the nasolabial angle to an approximately 90-degree angle.
The deviated and asymmetric nasal bone could be straighten out through an outer osteostomy.
To correct the plain and flat appearance of the glabella, we would use a silicone implant to transform the nose bridge into a straight line.

General Overview
The patient’s results were satisfactory. The preoperative issues the patient wanted to improve, droopy nose and deviated shape were enhanced.

Nasal Deviation Correction
Also, for the deviated nose correction, it is of great importance to determine whether the deviation is due to nasal bone asymmetry that makes the bone look bent, if it is due to deviated nasal cartilage, or if both the bone and the cartilage are bent. Nasal bone asymmetry can be corrected through an outer osteostomy , and in the case of deviated nasal cartilage, it is possible to perform a septal deviation corrective surgery and the nasal dorsum must be stretched through a spreader graft. Moreover, in the case that the joint between the nasal bone and the dorsum becomes bent, both can be done and using a silicone implant must be actively taken into consideration.
