1. Preoperative Design

This patient’s nose looked bent in general and they had gotten a nostril alar reduction surgery at another clinic. The patient visited my clinic to correct the parts that had developed asymmetry in the nostrils. They did not want to use a silicone implant, but they wanted to fix the bumpy and bent appearance of the nose bridge. During the design consultation, it was decided to slightly elevate the nose and correct the nasolabial angle.

2. A surgical Plan

To improve the bending parts of the nose bridge it was planned to perform an lateral osteotomy, and since there were some parts in the breathing area that were uncomfortable, we also planned a surgery to improve the nasal congestion. We decided to use the nose bridge self-rib cartilage bridge in the nose tip.

4. General Review

This was a surgery to correct nostrils that had become asymmetrical due to a nostril alar reduction surgery. The nostril alar reduction surgery is relatively easy but its complications are not simple. First of all, it results in having to carry scars that you do not want to see on both nostrils for the rest of your life, and as the nostrils had become smaller, they may also become asymmetrical. The worst thing is that they cannot be restored to their original state again. Once cut, the nostril cannot be rebuilt. This person also has nasal alar asymmetry, so through changing the position of the columella and relocating the nasal alar we tried to hide the nostril asymmetry as much as possible.
