Hello, this is Dr. Cha-Young Kang from Nose Lab Clinic.


Today, I would like to introduce a case of deviated septum and wide nose correction using autologous rib cartilage. The patient presented with both functional nasal obstruction and cosmetic concerns, including a deviated septum and a broad nasal shape.

1. Initial Evaluation for Deviated Septum and Wide Nose Correction

Frontal View:

  • Broad nasal base

  • Bulbous nasal tip

  • Visible deviation of the nose


Lateral View:

  • Mild dorsal hump

  • Droopy nasal tip

  • Depressed radix


Oblique View:

  • Mild maxillary protrusion, accentuating nasal imbalance


Basal (Nostril) View:

  • Nostril asymmetry

  • Septum deviated toward the left


2. Surgical Plan to Correct Deviated Septum and Wide Nose

Functional Correction:

  • Septoplasty to align the septum

  • Submucosal turbinate reduction using RF laser

  • Restoring symmetrical nasal airflow


Structural Reconstruction:

  • Lateral osteotomy to realign the nose

  • Septal extension with autologous rib cartilage

  • Bridge augmentation using autologous rib cartilage


Aesthetic Refinement:

  • Slimming the nasal contour

  • Controlled augmentation for natural height

  • Facial fat grafting for overall harmony

In this case, both structure and function were successfully restored using autologous rib cartilage. Lateral osteotomy corrected the deviation, and the use of rib cartilage ensured long-term stability and natural shaping. Facial fat grafting contributed to harmonious results across the entire midface.
