Today, I’d like to share a complex facial asymmetry correction case involving a patient who presented with a crooked nose, nostril asymmetry, and a depressed right nasal bone.

The main goal was to restore facial balance through osteotomy and septal reconstruction while maintaining a natural nasal tip height.

To achieve these outcomes, a step-by-step facial asymmetry correction plan was designed to address both functional and aesthetic issues simultaneously.

Restoring a Collapsed Nasal Bone and Correcting Septal Deviation in Facial Asymmetry


1. Osteotomy for the Depressed Right Nasal Bone

The patient’s right nasal bone was collapsed, causing noticeable facial asymmetry.

Through precise osteotomy, the depressed bone was realigned to restore natural contour and symmetry across the midface.


2. Correction of Septal Deviation

The crooked nose originated from an internally deviated septum.

We corrected the septal deviation to realign the nasal axis and prevent future deviation relapse—an essential step in stable facial asymmetry correction.


3. Septal Reconstruction with Autologous Rib Cartilage

Using the patient’s own rib cartilage, we reconstructed and extended the septum to reinforce structural support.

This preserved the patient’s desired nasal tip height while ensuring long-term balance and stability.


4. Chin Augmentation for Overall Facial Balance

To complement the upper facial corrections, subtle chin augmentation was performed to enhance symmetry and achieve a harmonious facial ratio.


5. Nasolabial Angle Adjustment

Adjusting the nasolabial angle softened the upper lip profile and improved the facial side line, contributing to an overall balanced appearance after facial asymmetry correction.
