Hello, this is Dr. Chayoung Kang, Director of Noselab Rhinoplasty Clinic. Today, we will introduce the corrective surgery case of a patient who had complex nasal issues such as an upturned, hooked and bulbous nose shape.
This patient had already gotten nose filler in 2017, making this an interesting case that involves a foreign body removal procedure as well.
Preoperative Design Consultation
An overall blunt and wide nose (bulbous nose)
Remaining filler of the glablella, making it look wide, and making the starting point look high.
Wide and rounded nose tip.
Irregular nose bridge (presumably caused by the filler)
Droopy and bold nose tip
High nose bridge starting point
Prominent nasal hump
The nasal hump is more visible
Noticeable droopy nose tip
The general shape looks natural but the nose tip looks wide and blunt.
Surgical Plan
Taking into account the patient’s requirements and condition, we made the following surgical plan:
Foreign body removal (filler)
Septal extension using the septal cartilage and donated rib cartilage.
Bulbous nose correction
Improvement of the droopy nose tip
Lowering of the nasal bridge starting point
Before and After Surgery Photos
The nose that looked blunt and wide became slimmer and its appearance became sophisticated.
The wide appearance of the glabella disappeared and the starting point became more natural-looking.
Delicatedly changed the round and wide apperance of the nose tip to be more three-dimensional.
Improvement of the bumpy nose bridge into a smooth nose shape.
Natural-looking correction of the nasal hump into a soft curve.
Facial balance improvement by adjusting and raising the droopy nose tip.
Lowering of the nasal bridge starting point making the nose shape look like a natural straight line.
Improvement of the hooked nose into a softer nose shape.
The overall appearance of the face became more harmonious by correcting the droopy nose tip.
Correction of the blunt and wide nose tip into a beautiful and sophisticated appearance.
Overall the nostril shape became more balanced.

Overview
This case is one that simultaneously solved complex problems such as a deviated, hooked, and bulbous nose. It was an especially meaningful surgery because it also removed foreign substances from previous filler treatments.
Septal lengthening surgery using septal cartilage and donated costal cartilage played a major role in correcting the droopy nasal tip and strengthening the overall structure of the nose. This improved the droopiness of the nasal tip and at the same time reduced the width that is characteristic of a bulbous nose.
The nasal hump correction was not simply to remove the protruding parts, but was carefully performed to harmonize with the overall nose shape. As a result, a we created a beautiful nose line with soft curves.
Lowering the starting point of the bridge of the nose and improving the wide appearance of the glabella area played an important role in balancing the overall face. This allowed for a more natural and harmonious image to be created.
In particular, in this surgery, we successfully implemented the patient’s request of “a straight nose with a natural yet gorgeous appearance.” This can be said to be the result of accurately understanding and realizing the patient’s aesthetic needs, rather than simply correcting the problem.
By using a closed rhinoplasty approach, we were able to shorten the postoperative recovery period and complete the surgery without leaving a scar, which greatly contributed to increasing patient satisfaction.
