In this case, we addressed both the functional and cosmetic aspect at the same time.
Preoperative Design Consultation
Among those suffering from nasal congestion, there are many who are also dissatisfied with their nose shape.
Frontal view:
An overall wide nose (bulbous nose)
Prominent blunt and broad nose tip
Flat glabella
Noticeable nostril asymmetry

Droopy and blunt nose tip

The nasolabial angle was smaller than 90 degrees, causing a protrudifng mouth.
45-degree angle
The bluntness and droopiness of the nose tip is more noticeable.
The glabella is flat and plain.
Nostril view:
Uptilted nose tip and wide nostrils on both sides.
Visible nostril asymmetry (the left nostril looks smaller than the right side nostril)
Functional Issues:
The patient complained of frequent nasal congestion symptoms.
Septal upper deviation and hypertrophic rhinitis were diagnosed through the CT scan.
Through the intranasal endocopy, we observed the mucosal membrane of the inferior turbinates appeared pale and thickened, which indicated an allergic rhinitis diagosis.
Surgical Plan
We came up with a plan after determining the patient's condition and requests for the surgery, as follows:
Functional improvement plan
Septoplasty
Inferior turbinates submucosal resection surgergy using a high-frequency laser.
Cosmetic improvement plan
Septal extension graft using autologous rib cartilage.
Partial removal of the subcutaneous tissue of the nose tip.
Alar relocation and cartilage tying.
Raising the nose bridge with a silicone implant.

Contour enhancement of the general shape of the nose.
Transformed the plain and flat nasal dorsum into a natural-looking straight line.
Made a natural-looking connection of the previously depressed glabella and the nasal bridge.
Improvement of the nasolabial angle by slightly lowering the columella and raising the nose tip.
Noticeable improved effect on the mouth protrusion.
Improvement of the overall nose shape by naturally raising the previously droopy nose tip.
Achieved a balanced contour between the height of the nose bridge and the nose tip projection.
Correction of the nostril asymmetry.
Arranged the contour of the nose tip.
The shape and size of the nostrils became more balanced.

Overview
This case successfully achieved both functional and cosmetic improvements. While solving functional problems such as nasal congestion, we were also able to improve cosmetic issues such as a flat and wide nose, with a low nasal bridge.
Through septoplasty and submucosal resection of the inferior turbinates, the nasal obstruction symptoms improved, and at the same time, the blunt and wide nose shape was improved through septal extension using autologous costal cartilage and nose tip correction. By raising the bridge of the nose using silicone implants, we achieved the overall balance of the nose and satisfactory cosmetic and functional results.
In particular, by using a closed rhinoplasty approach, we were able to shorten the postoperative recovery period and complete the surgery without leaving external visible scars, which greatly contributed to increasing patient satisfaction.
