Hello, this is Dr. Chayoung Kang, Director of Noselab Rhinoplasty Clinic.

Today, we will introduce a corrective case of no-implant rhinoplasty for a patient with a short, blunt, and humped nose. The patient’s main concerns were a wide nose tip, a short and upturned appearance, and nasal asymmetry.

Surgical Details – No-Implant Rhinoplasty

The following surgeries were performed through closed rhinoplasty.

a) Autologous rib cartilage and transplant.

Sufficient amount of cartilage was harvested for nose bridge and tip surgery.

b) Septal graft extension surgery:

Extension and strenghtening of the septal cartilage by using the self-rib cartilage.
This makes a short nose look naturally longer and corrects the upturned nose tip.

c) Correction of septal deviation:

The septal deviation correction also improved the functional aspect of the nose.

d) Nasal hump correction.

Trimming of the protruding nasal bone to correct the nasal hump.

e) Heightening of the nasal bridge.

Raising the nasal bridge to match the tip of the nose using autologous costal cartilage.

f) Alar cartilage repositioning:

Repositioning of the alar cartilage to improve the blunt nose tip.
This has helped to sharpen the outline of the tip of the nose, improving its overall appearance.

All procedures were performed using a closed approach, completing the surgery without external scars.

The nose, which used to look blunt and wide, has been improved to a slim and sophisticated shape.

The nasal hump was corrected to be smooth straight line.
The high nose tip has appropriately been lowered, and the overall shape became more harmonious.

Overview

This case is one where we solve complex problems such as a blunt and short nose and a humped nose at the same time. In particular, we were able to achieve natural results through a no-implant rhinoplasty surgery as the patient requested.

No-implant rhinoplasty is a surgical method that maximizes the use of the individual’s nose bridge. Therefore, it may not be recommended for cases where the original nose bridge is too low or severely deviated. Therefore, it is not a surgical method suitable for everyone, and should be chosen carefully according to the individual’s condition.

Many people may mistakenly think that no-implant rhinoplasty is a surgery that does not insert anything into the bridge of the nose because of its name. In fact, no-implant surgery uses autologous tissues such as cartilage, fascia, and dermis instead of artificial implants such as silicone. These tissues go through a process called engraftment over time, and some of them may be absorbed during this process.

Depending on the type of tissue, the absorption rate is different, so the prediction rate drops significantly. Therefore, there is a disadvantage that the nasal bridge may become flatter or bumpy after 6 months at the minimum and several years at the maximum. Therefore, patients should fully understand these characteristics and choose an appropriate surgical method.

Currently, non-implant surgeries are not yet a perfect replacement for silicone implants from an aesthetic standpoint. However, they can be a good option for patients who want natural results.

In this surgery, we corrected and lowered the short and upturned nose through septal extension using autologous costal cartilage. In addition, the blunt nose tip improved through alar cartilage repositioning, and the nasal hump appearance changed into a smooth nose shape.
