Hello, this is Dr. Cha-Young Kang, Director of Nose Lab Clinic.

Today, I’m sharing a revision rhinoplasty with autologous rib cartilage case, where a patient underwent nasal reconstruction to correct a deviated nose and improve a protruding mouth. This case highlights the importance of structural stability and aesthetic balance in complex revision rhinoplasty.

1. Surgical History

  • 2009: Silicone implant + Septal cartilage graft.

2. Initial Assessment

Frontal View

  • Noticeable nasal deviation.

  • Implant translucency in the nasal bridge.

  • Bulbous and wide nasal tip.

  • Pinched appearance on the left side of the nasal tip.

  • Nostril asymmetry.

Side View

  • Elongated nose structure.

  • Visible protrusion of the upper lip.

  • Drooping nasal tip.

Oblique View

  • Overall disharmony in facial proportions.

  • Prominent upper lip projection.

Functional Issues

  • Mucus retention causing nasal discomfort.

3. Surgical Approach – Revision Rhinoplasty with Autologous Rib Cartilage

Removal of Existing Material

  • Safe removal of the Gore-Tex previously used in the nasal tip.

  • Careful excision of damaged tissue.

Structural Reconstruction

  • Reinforcement of nasal framework using autologous rib cartilage.

  • Septal extension graft for improved nasal support.

  • Nasolabial angle correction using rib cartilage for enhanced harmony.

Aesthetic Refinement

  • Straightening of the deviated nasal bridge.

  • Refinement of the nasal tip for a natural look.

  • Achieving nostril symmetry.

4. Postoperative Results

Frontal View

  • Corrected nasal deviation.

  • Resolution of implant translucency.

  • Well-defined and natural-looking nasal tip.

  • Elimination of nasal tip pinching.

  • Symmetric nostril shape.

Side View

  • Improved nasolabial angle, reducing the protruding mouth effect.

  • Softer and more natural nasal contour.

  • Adjusted nasal length for better proportions.

  • Correction of the drooping nasal tip.

Oblique View

  • Balanced facial contours.

  • Harmonious nasal and upper lip projection.

  • Smooth nasal bridge transition.

5. Expert Insights

This case required the careful removal of Gore-Tex and precise reconstruction using autologous rib cartilage due to severe septal cartilage damage. Structural stability was the top priority, ensuring long-term nasal support while achieving the natural aesthetics the patient desired.

Key takeaways from this case:

✅ Safe removal of problematic implants (Gore-Tex).

✅ Reinforcement with autologous rib cartilage for durability.

✅ Balance between nasal structure and facial harmony.

✅ Functional and aesthetic improvements in one surgery.
