1. Preoperative consultation
This patient visited my clinic to correct a blunt and upturned nose with nostril asymmetry.
2. Surgery Plan
I made a plan to correct the blunt appearance of the nose tip through the removal of the scar tissue and the relocation of the alar cartilage, and through reconstructing the septal cartilage and moving it to the left side; I also planned to fix the nasal congestion and the nostril asymmetry.
3. Surgical findings
The previous clinic had excessively collected septal cartilage, including the bone parts to a point where the septum was weak, and the septal deviation was worsening. The remaining parts of the septal cartilage had sustained severe damage, thus I reconstructed it by using the patient´s own costal cartilage. The damage was directly related to the bending of the nose tip, the asymmetry of the nostrils and the nasal congestion.

5. General Review
In this revision rhinoplasty’s blunt nose tip correction, it is important to remove the damaged tissue around the nose tip and to relocate the alar. In particular, the delicate removal of the subcutaneous tissue (contracture tissue, scar tissue) is important in the revision surgery, since they are related to the inflammation that occurred 6 months prior. Also, in the case of severe alar damage, it can be a cause for nostril asymmetry. In the previous surgery that the patient had in another hospital, they also used their own rib cartilage, and 3 months later they had a revision surgery. After the patient had received stem cell therapy, I used the self-rib to create a nice nose shape.
