This case demonstrates how long-standing fillers can distort nasal structure, leading to both aesthetic and functional concerns. Rib cartilage grafting provided the necessary support to refine the nasal tip and improve contour.

Lateral osteotomy narrowed the widened bridge, while dorsal hump and alar base reduction enhanced the nose’s proportions. Functional techniques, such as septoplasty and turbinate reduction, resolved breathing issues and nasal congestion.

Through a comprehensive closed rhinoplasty approach, we achieved a sleeker, more balanced nasal shape that fulfilled both visual and functional expectations.
