Preoperative Analysis – Bulbous Nose Correction and Nasal Asymmetry in Closed Rhinoplasty Korea


This patient visited my clinic for bulbous nose correction to refine a wide nasal tip and a flat, curved bridge.

During the preoperative consultation, I identified that her nasolabial angle was smaller than 90 degrees, which made her mouth appear slightly protruding.

The goal was to correct this by adjusting the nasolabial angle to approximately 95 degrees, repositioning the columella downward, and designing the nasal bridge to start naturally around the pupil line.

From the frontal view, the nose appeared slightly wide relative to the intercanthal distance.

To correct this proportion and improve nasal symmetry, I planned nasal base reduction and lateral osteotomy to correct the wide and curved nasal bones.

The patient also had congenital nostril asymmetry, with the left nostril slightly larger and outward.

Therefore, I planned to reposition the alar cartilage and reduce the nasal base to achieve more balanced nostril symmetry.

Surgical Findings – Donated Rib Cartilage Use in Closed Rhinoplasty Korea


The patient also reported mild nasal obstruction, and CT and nasal endoscopy revealed a left-sided septal deviation.

Accordingly, septoplasty and rhinitis surgery were performed simultaneously.

Intraoperatively, the septal cartilage was found to be very thin and small, insufficient for nasal tip reconstruction.

Therefore, I used donated rib cartilage combined with the patient’s septal cartilage for a more stable nasal tip framework.


The internal airway was significantly widened, and the operation proceeded smoothly without complications.

This approach allowed for both functional and aesthetic improvement, which is one of the key goals of Closed Rhinoplasty Korea.


Postoperative Results – Natural Line and Balanced Proportion After Bulbous Nose Correction


Immediately after surgery, the bulbous and wide tip was refined, and the columella descended naturally.

The nasolabial angle was corrected to approximately 95 degrees, creating a soft and balanced contour.

By aligning the implant’s starting point with the pupil line, the overall nasal bridge achieved a more natural height and smooth transition from the forehead to the tip.


The bulbous tip and wide nasal base were significantly improved.

Even the nostril asymmetry—which was congenital—was corrected effectively, creating a natural oval shape with harmonious proportions.

Surgical Outcome and Long-Term Expectations


This case of bulbous nose correction in Closed Rhinoplasty Korea was performed using donated rib cartilage for structural stability.

Because the patient’s skin was relatively thick, I carefully reduced subcutaneous tissue around the tip to prevent stiffness and ensure a natural contour.

A smooth, straight, and three-dimensional nasal line was achieved, and immediate postoperative results were excellent.


If postoperative scar management is continued for six months, the result will maintain both a refined appearance and stable internal structure.

This surgery demonstrates how Closed Rhinoplasty Korea can address both bulbous and asymmetrical nose shapes effectively, achieving long-term functional and aesthetic harmony.
