Contracted Nose and Upturned Tip Revision Rhinoplasty – Case in Korea

Hello, this is Dr. Cha-Young Kang, head surgeon at Nose Lab Clinic.


Today, I’d like to share a contracted nose revision rhinoplasty case in Korea involving multiple concerns: contracted nose, bulbous tip, upturned nose, short nose, and nostril asymmetry. Given the patient’s history of repeated surgeries, this was a particularly challenging revision.

Patient’s surgical history:

  • 2019: Tipplasty with ear cartilage + philtrum reduction

  • Early 2022: Dorsal/tip surgery with autologous rib cartilage

  • Mid 2022: Implant removal after inflammation

  • Late 2022: Tip surgery with rib cartilage + paranasal augmentation

  • 3 sessions of stem cell therapy at Nose Lab Clinic

Main concerns before surgery:

  • Upturned nasal tip

  • Excessive nostril show

  • Bulbous, wide tip

  • Nostril asymmetry

  • Large nasolabial angle

  • Prominent columella


Surgical Procedure in Contracted Nose and Upturned Tip Correction

The surgery was performed using a closed (endonasal) approach without external scarring.


a) Autologous Rib Cartilage Grafting – Rib cartilage harvested for septal extension, providing stability and correcting the upturned tip.


b) Nostril Lowering Surgery – Rib cartilage used to lower the nostrils and improve symmetry.


c) Dorsal Augmentation – Smooth dorsal line achieved with silicone implant shaped to fit the patient’s facial balance.


d) Tip and Columella Correction – Tip lowered, columella adjusted, and nasolabial angle reduced.


e) Refinement of Bulbous Tip – Wide tip reshaped into a more refined contour.


f) Nostril Asymmetry Correction – Achieved through septal extension, cartilage repositioning, and suturing.

This contracted nose revision rhinoplasty demonstrates how even complex cases with inflammation history can be corrected. Removal of contracted tissue and reconstruction with rib cartilage provided long-term stability.
