Hello, this is Dr. Cha-Young Kang, Director of Nose Lab Clinic.


Today, I’m sharing the case of a patient who sought correction for a dorsal hump and a dynamic drooping tip—commonly referred to as an "arrow nose"—through closed rhinoplasty in Korea. This hump nose correction and drooping tip correction procedure became particularly effective when smiling. Our goal was to achieve a refined, natural nasal tip and address both aesthetic and functional concerns.

Functional Issues

  • Chronic rhinitis symptoms

✨ Patient Goals

  • Correct dorsal hump and deviated nose

💉 Surgical Approach

Structural Correction

  • Used autologous rib cartilage (due to underdeveloped septal cartilage) for nasal tip support

  • Corrected dorsal hump

  • Performed osteotomies to realign deviated nasal bones

  • Lifted the drooping nasal tip


Tip-Specific Refinement

  • Reshaped the bulbous tip for a slimmer profile

  • Addressed the dynamic "arrow nose" issue during facial expression

  • Balanced nasal length and projection

  • Created a naturally contoured nasal tip


Functional Correction

  • Septoplasty and submucosal resection of the inferior turbinates using radiofrequency ablation

  • Improved breathing and relieved nasal obstruction

This patient presented with classic concerns of a prominent dorsal hump, deviation, and dynamic arrow nose deformity—issues that are not only aesthetically challenging but also complex to treat. The arrow nose effect, which causes the nasal tip to dramatically droop when smiling, is typically the result of combined structural laxity and muscular action. It often cannot be resolved with a simple rhinoplasty alone.


The cornerstone of this procedure was precise tip reconstruction using autologous rib cartilage to replace insufficient septal support. By carefully repositioning and shaping the tip cartilage, we prevented exaggerated movement during facial expression while simultaneously slimming and refining the nasal contour.


Osteotomies and hump reduction allowed for realignment of the deviated nose and correction of the exaggerated dorsal line, while subtle radix lowering softened the overall expression. Functional procedures addressed chronic nasal congestion and improved the patient’s breathing—especially crucial given her worsened symptoms in recent months.


Ultimately, we achieved a stable, natural-looking result that maintained balance at rest and during expression, correcting both the visible deformities and underlying functional impairments.

This was a successful example of how detailed structural reconstruction and dynamic analysis can produce outcomes that are both aesthetically harmonious and functionally sound.
