Today, I’d like to share a case of droopy nasal tip correction and wide nose refinement using autologous rib cartilage after multiple previous surgeries.
Patient History and Reasons for Droopy Nasal Tip Correction

The patient had undergone several procedures:
2010: Primary rhinoplasty (silicone + septal cartilage)
2019: Orthognathic (jaw) surgery
2022: Revision rhinoplasty (silicone + dermal graft)
After jaw surgery, facial bone repositioning made the nose appear wider and droopier.Even after revision surgery, the droopy nasal tip persisted and nasal obstruction remained due to chronic rhinitis.

The patient’s goals were to:
Lift and refine the drooping nasal tip
Correct nasal width caused by orthognathic surgery
Restore overall facial balance
Improve breathing and relieve nasal congestion

Revision Rhinoplasty with Autologous Rib Cartilage
Removed prior silicone and dermal grafts, then rebuilt the nasal framework using the patient’s rib cartilage for long-term stability.
Septal Reconstruction
Straightened the nasal axis and reinforced support to maintain tip projection after droopy nasal tip correction.
Alar Base Reduction
Narrowed the nasal width to improve frontal balance and harmony with post-jaw-surgery features.
Functional Enhancements Septoplasty and submucosal resection of inferior turbinates to improve breathing and relieve rhinitis symptoms.
Surgical Results and Facial Balance

The new nasal shape harmonized with the altered facial structure after jaw surgery.

The droopy nasal tip was naturally elevated, creating a smooth and elegant contour.

Breathing improved significantly, and nasal obstruction was reduced.
