Hello, this is Dr. Cha-Young Kang, head surgeon at NoseLab Plastic Surgery in Seoul.

Today, I’d like to share a case of droopy nasal tip correction and wide nose refinement using autologous rib cartilage after multiple previous surgeries.

Patient History and Reasons for Droopy Nasal Tip Correction

Preoperative consultation photos for droopy nasal tip correction – frontal, 45-degree oblique, and side views.

The patient had undergone several procedures:

  • 2010: Primary rhinoplasty (silicone + septal cartilage)

  • 2019: Orthognathic (jaw) surgery

  • 2022: Revision rhinoplasty (silicone + dermal graft)

After jaw surgery, facial bone repositioning made the nose appear wider and droopier.Even after revision surgery, the droopy nasal tip persisted and nasal obstruction remained due to chronic rhinitis.

The patient’s goals were to:

  • Lift and refine the drooping nasal tip

  • Correct nasal width caused by orthognathic surgery

  • Restore overall facial balance

  • Improve breathing and relieve nasal congestion

Surgical Plan – Droopy Nasal Tip Correction with Rib Cartilage

  1. Revision Rhinoplasty with Autologous Rib Cartilage

Removed prior silicone and dermal grafts, then rebuilt the nasal framework using the patient’s rib cartilage for long-term stability.

  2. Septal Reconstruction

Straightened the nasal axis and reinforced support to maintain tip projection after droopy nasal tip correction.

  3. Alar Base Reduction

Narrowed the nasal width to improve frontal balance and harmony with post-jaw-surgery features.

  4. Functional Enhancements Septoplasty and submucosal resection of inferior turbinates to improve breathing and relieve rhinitis symptoms.

Surgical Results and Facial Balance

Before and immediately after droopy nasal tip correction – frontal view comparison.
Before Surgery (Left) / Immediately After Surgery (Right)
  • The new nasal shape harmonized with the altered facial structure after jaw surgery.

Before and immediately after droopy nasal tip correction – side view comparison showing natural tip elevation.
Before Surgery (Left) / Immediately After Surgery (Right)
  • The droopy nasal tip was naturally elevated, creating a smooth and elegant contour.

Before and immediately after droopy nasal tip correction – oblique side view comparison showing refined nasal contour.
Before Surgery (Left) / Immediately After Surgery (Right)
  • Breathing improved significantly, and nasal obstruction was reduced.

Before and immediately after droopy nasal tip correction – nostril view showing improved symmetry and nasal base shape.

Expert Insights on Droopy Nasal Tip Correction

Orthognathic surgery changes the facial skeleton, sometimes making the nose appear smaller or wider.

In this case, previous grafts and scar tissues were carefully removed, and the nose was reconstructed with autologous rib cartilage to achieve both strength and aesthetics.

Proper droopy nasal tip correction requires evaluating prior surgical changes and restoring a stable framework.

At NoseLab Plastic Surgery, we focus on closed rhinoplasty techniques to achieve natural, durable, and functional results.
