Large Nose Correction: Balanced Facial Design with Closed Rhinoplasty


Hello, I’m Dr. Cha-Young Kang, Director of Nose Lab Clinic.


Today, I will introduce a case of large nose correction where improving the nasal structure helped enhance the patient’s overall facial harmony and proportions.


Patient’s Requirements

  • Reduce the size and prominence of the nose

  • Design a nose that harmonizes with the patient’s face

  • Complementary procedures: autologous fat grafting and chin augmentation

Pre-operative Analysis

Frontal view:

  • Bulbous nasal tip

  • Long and large nose

  • Deviated nasal bridge

Side profile view:

  • Droopy nasal tip

  • Hooked bridge

  • Prominent nasal projection

  • Recessed chin

Nostril and nasal base:

  • Wide nasal base

  • Asymmetric nostrils

Surgical Plan

Nasal structure correction:

  • Outer osteotomy for narrowing

  • Tip correction and relocation with autologous rib cartilage

  • Nasal base reduction

  • Septal deviation correction

Overall facial balance:

  • Chin augmentation to correct retrusion

  • Autologous fat grafting to restore volume and enhance facial contour

Expert’s Opinion

There are physical limitations in making a large nose dramatically smaller. However, by focusing on the harmony of the entire face, it is possible to create the impression of a smaller, refined nose.


In this case, we did not only reshape the nose but also addressed overall facial proportions. By performing autologous fat grafting to supplement depressed areas and chin augmentation to enhance projection, we achieved a balanced three-dimensional effect. As a result, the patient’s nose appeared significantly smaller and more harmonious with the rest of the face.
