Long and Droopy Nose Correction with Self-Rib Cartilage and Nose Bump Removal

Hello, this is Dr. Chayoung Kang, Director of Noselab Clinic, specializing in closed rhinoplasty in Korea that prioritizes natural beauty and minimal scars.


Today, I will introduce a real case of long and droopy nose correction combined with nose bump removal using a closed rhinoplasty technique.


Preoperative Consultation and Nasal Analysis


The patient visited our clinic to correct a long nose with a noticeable bump.

Despite undergoing a previous surgery at another hospital, the nasal hump remained, and the droopy nose tip continued to give the impression of a long, downward-sloping nose.


Preoperatively, the patient’s nasal bridge was high but unbalanced, and the columella and tip projection made the lower third of the face look elongated.


Surgical Plan for Long and Droopy Nose Correction


To correct the long and hooked nose, an outer osteotomy was planned to remove the bump and realign the nasal bone.

The droopy nasal bridge was gathered and reshaped, and the nose tip was lifted firmly using autologous rib cartilage (self-rib).

The droopy columella was also reduced for a more natural angle.


Since the skin of the nasal tip was thin, we used cartilage grafts for tip reinforcement and artificial dermis to soften the contour and prevent visible cartilage outlines.


Surgical Technique and Key Considerations


For a long nose correction, it is crucial to shorten the nasal bone slightly while lifting the tip.

To fix the hooked nose, both the nasal bone and upper lateral cartilage were partially trimmed, and a thin silicone implant was inserted to create a smooth and straight dorsal line.

The columella was also straightened to ensure central alignment.


A hooked or bumpy nose often makes the nose look longer, so correcting the bump simultaneously improves both the nasal profile and perceived length.

Postoperative Overview and Results


Photos were taken immediately after surgery with the patient’s consent.After the procedure, the hump was removed, the droopy tip lifted, and the overall nasal line appeared shorter and more refined.


The nasal bone and upper cartilage were reshaped precisely through outer osteotomy, preventing “open roof” deformities and maintaining a natural slope.

The final result shows a balanced, harmonious nasal shape that looks natural from all angles.
