Case of Male Revision Rhinoplasty – Correcting Contracted and Upturned Nose in Korea

Hello, this is Dr. Cha-Young Kang, head surgeon at Nose Lab Clinic.


Today, I’d like to introduce a male revision rhinoplasty case in Korea involving a patient with a contracted nose, upturned tip, shortened nasal length, and nostril asymmetry following multiple previous rhinoplasty procedures. This case demonstrates how careful planning and advanced surgical techniques can restore both function and aesthetics.


Pre-Surgery Condition of Male Revision Rhinoplasty


Patient’s surgical history:

  • 2017: Septoplasty + Silicone implant

  • 2020: Silicone + Autologous rib cartilage graft

  • Dec 2023: Silicone + new autologous rib cartilage graft

  • 3 months later: Tip swelling and balloon-like redness → incision + implant removal

Patient’s requests:

  • Correction of nasal obstruction (occasional loss of taste)

  • Reconstruction of nasal structure

  • Creation of a straight nasal line

  • A refined, masculine nasal appearance

Clinical findings:

  • Upturned nasal tip

  • Nostril asymmetry and excessive nostril show

  • Wide, curved nasal bridge

  • Short and droopy nose

  • Depressed scar tissue on nasal tip and columella


Surgical Plan for Upturned Nose and Contracted Nose Correction

The surgery was performed through a closed (endonasal) approach without external scars.


a) Stem Cell Injection Therapy – 5 sessions pre-surgery + 6th on the day of surgery to improve tissue elasticity and reduce recurrence of contraction.


b) Rib Cartilage Harvesting – Taken from the left side to provide sufficient structural material.


c) Septal Cartilage Reconstruction – Rib cartilage used to reconstruct the septum and correct deviation for better breathing.


d) Nostril Correction – Nostril lowering to balance asymmetry and reduce nostril exposure.


e) Nasal Tip Correction – Septal extension with rib cartilage to lower the upturned tip and adjust the columella.


f) Nasal Bridge Correction – Slim, straight dorsal line created with silicone implant.


g) Scar Tissue Improvement – Dermis and cartilage grafted beneath nasal tip skin to smooth surface irregularities.

This male revision rhinoplasty addressed a contracted nose, upturned tip, short nose, and nasal scarring. Stem cell therapy was critical in preparing the tissue, while rib cartilage provided durable structural support.
