Surgical History and Patient’s Long Journey with Revision Rhinoplasty


Between 2009 and 2014, the patient underwent five rhinoplasty surgeries using septal cartilage, ear cartilage, donor rib cartilage, autologous rib cartilage, and silicone implants. Despite multiple attempts, her nose appeared increasingly artificial. She also received filler injections eight years ago.

Patient’s Goals for Revision Rhinoplasty


The patient hoped for:

  • A naturally refined, elegant nose

  • A straight bridge with a subtle curve

  • Correction of the upturned tip

  • Improved nostril symmetry


Surgical Plan – Complex Revision Rhinoplasty Strategy


Our surgical plan included:

  • Septal Extension Graft: Correcting the upturned tip and shaping the nasal tip precisely

  • Reuse of Existing Autologous Tissue: Incorporating rib, septal, and ear cartilage

  • Additional Rib Cartilage Harvesting: Ensuring stable septal reconstruction

  • Removal of Foreign Substances: Clearing filler and silicone

  • Customized Silicone Implant: Positioned slightly lower for a natural bridge


Due to severe septal cartilage damage from prior surgeries, we reconstructed the septum with autologous rib cartilage—an essential step in this complex revision rhinoplasty.
