Surgical History and Patient’s Long Journey with Revision Rhinoplasty
Between 2009 and 2014, the patient underwent five rhinoplasty surgeries using septal cartilage, ear cartilage, donor rib cartilage, autologous rib cartilage, and silicone implants. Despite multiple attempts, her nose appeared increasingly artificial. She also received filler injections eight years ago.
Pre-Surgical Analysis and Design Consultation for Revision Rhinoplasty
Our consultation revealed several key issues:

Upturned nasal tip
High starting point of the bridge
Artificial, arched “soft bridge” shape
Nostril asymmetry and scars from philtrum and alar reduction
Columella scarring from multiple open rhinoplasty procedures
These findings emphasized the need for a tailored revision rhinoplasty approach.

Patient’s Goals for Revision Rhinoplasty
The patient hoped for:
A naturally refined, elegant nose
A straight bridge with a subtle curve
Correction of the upturned tip
Improved nostril symmetry
Surgical Plan – Complex Revision Rhinoplasty Strategy
Our surgical plan included:
Septal Extension Graft: Correcting the upturned tip and shaping the nasal tip precisely
Reuse of Existing Autologous Tissue: Incorporating rib, septal, and ear cartilage
Additional Rib Cartilage Harvesting: Ensuring stable septal reconstruction
Removal of Foreign Substances: Clearing filler and silicone
Customized Silicone Implant: Positioned slightly lower for a natural bridge
Due to severe septal cartilage damage from prior surgeries, we reconstructed the septum with autologous rib cartilage—an essential step in this complex revision rhinoplasty.
Immediate Post-Surgical Results
Side View: Lower, more natural bridge and corrected nasolabial angle

Oblique View: Elegant, cohesive profile

Frontal View: Straighter, natural bridge and improved nostril balance

Nostril View: Reduced asymmetry and refined nostril appearance

Conclusion – The Importance of a Tailored Revision Rhinoplasty
This 6th revision rhinoplasty case underscores the importance of personalized planning and expertise. Multiple open surgeries often leave scar tissue, which makes further open approaches risky. Closed rhinoplasty can be a safer, more refined option in such complex cases.
At Nose Lab Clinic, we are committed to achieving safe, natural, and satisfying outcomes through structural, closed techniques.
Nose Lab Clinic performs all surgeries using a closed rhinoplasty approach.
