Hello, this is Dr. Cha-Young Kang, Director of Nose Lab Clinic.


Today, I’m sharing a case of revision rhinoplasty after hump removal, where the patient had a crooked nasal bridge, bulbous tip, and asymmetry despite two previous surgeries. Our approach aimed to improve nasal aesthetics and function while maintaining a natural profile.

📍 Surgical Background

Initial Surgical History

  • 20 years ago: Hump removal surgery in the U.S.

  • 6 years ago: Septoplasty + dorsal hump correction + osteotomy


Key Deformities Identified Before Surgery

  • Frontal View: Crooked nasal bone, wide width, bulbous and undefined tip

  • Side View: Drooping tip, residual hump, blunt contour

  • Nasal Base: Columella deviation to the left, asymmetrical nostrils

1. Frontal View

  • Crooked nasal bone

💉 Revision Rhinoplasty Plan

1. Structural Correction – Osteotomy and Rib Cartilage

  • Lateral osteotomy to straighten the bone

  • Septal extension graft using autologous rib cartilage

  • Dorsal height preserved to avoid over-elevation


2. Aesthetic Goals – Tip Refinement and Nasal Symmetry

  • Elevated, projected nasal tip for balance

  • Tip grafting and reshaping for a slimmer look

  • Nasolabial angle correction and columella realignment

  • Nostril symmetry improvement


3. Functional Enhancements – Septoplasty and Breathing Improvement

  • Septoplasty for deviated septum

  • Radiofrequency turbinate reduction for allergic rhinitis

  • Noticeable improvement in breathing quality

This case shows the importance of strategic revision after failed hump removal. By combining osteotomy, rib cartilage grafts, and nasal base reshaping, we achieved both functional and aesthetic success.


The patient desired a natural result—not over-elevated or artificial. We preserved moderate dorsal height, reshaped the tip, and corrected breathing issues.
