1.Surgical Background – Why Secondary Rhinoplasty After Silicone Removal Was Needed
This case focuses on secondary rhinoplasty after silicone removal, performed on a patient who had undergone multiple nasal surgeries over the past 30 years. The most recent procedure involved the removal of a long-standing silicone implant, which had left the nose widened and structurally compromised.

2. Initial Assessment – Post-Silicone Removal Nasal Condition

Frontal View
Bulbous nasal tip
Mild nasal deviation
Pinched alar base
Widened nasal bridge due to implant removal
Side Profile
Undefined, droopy nasal tip
Depressed dorsum
Slightly protruding mouth
Small nasolabial angle
Nostril View
Noticeable asymmetry
Structural imbalance in alar base and nostrils
3. Patient’s Goals in Secondary Rhinoplasty After Silicone Removal
✔ Correct nasal deviation
✔ Achieve a refined, feminine tip
✔ Improve nostril symmetry
✔ Shorten the appearance of the nose (avoid over-projection)
4. Functional Symptoms
Chronic rhinitis
Frequent nasal itching

5. Surgical Plan – Strategy for Secondary Rhinoplasty After Silicone Removal
Structural Reconstruction
Rib cartilage graft for foundational support
Correction of deviated nasal axis
Use of silicone implant for dorsal augmentation
Tip Refinement
Reduction of bulbous tip
Creation of a more feminine and defined nasal tip
Correction of the pinched alar area
Nostril and Base Adjustment
Symmetry correction of nostrils
Balancing overall nasal proportions
Enhancing the nasolabial angle for a natural profile
6. Postoperative Outcomes

Frontal View
Straighter nasal bridge
Softer and more refined nasal tip
Balanced nasal structure

Side Profile
Natural dorsal height
Feminine and elegant tip definition
Harmonized nasolabial angle

Nostril View
Improved symmetry
Smoothed curvature and natural contour

7. Expert Insight – Key Points in Secondary Rhinoplasty After Silicone Removal

This was a challenging case of secondary rhinoplasty after silicone removal, as the patient had undergone several previous surgeries. The biggest concerns were structural instability, tip refinement, and asymmetry correction after the implant had been removed.
By combining autologous rib cartilage and a carefully selected silicone implant, we were able to reconstruct the nasal framework, correct alignment, and produce a naturally refined, feminine nasal shape.
Nose Lab Clinic performs all surgeries using a closed (scarless) approach.
