Hello, this is Dr. Cha-Young Kang, Director of Nose Lab Clinic.

Today, I’m sharing a real case of straight-line nose surgery performed using autologous rib cartilage on a patient with a bulbous, slightly upturned nose and a short nasal profile. Her goal was to achieve a chic, elevated straight nasal line, often referred to as a “jikban-line”, while maintaining natural harmony with her facial structure.

🔎 Initial Assessment – Upturned Nose with Bulbous Tip and Lack of Projection

Frontal View Observations

  • Rounded and wide nasal tip

  • Slightly upturned nasal shape

  • Wide nasal bridge

Side View Observations

  • Short nasal length

  • Low nasal dorsum

  • Tip overly lifted, mouth protrusion visible

Patient Skin Characteristics

  • Thick nasal tip skin

  • Limited elasticity

  • Average-width nasal base

✨ Surgical Goals – Enhancing the Nasal Line through Straight-Line Nose Surgery

  • Create a refined and elevated straight-line profile

  • Extend the nasal tip with structural stability

  • Lower the columella for aesthetic balance

  • Achieve natural harmony without tension on the skin

💉 Surgical Technique – Rib Cartilage for Straight-Line Nose Surgery

Tip Reconstruction

  • Septal extension using autologous rib cartilage

  • Corrected upturned nose and enhanced tip projection

  • Lowered the columella

Dorsal and Nasal Bridge Refinement

  • Sculpted a smooth, straight nasal dorsum (jikban-line)

  • Performed osteotomy to refine the wide nasal bridge

  • Adjusted nasolabial angle to reduce mouth protrusion appearance

Customized for Thick Skin

  • Tailored the framework to accommodate low skin stretchability

  • Ensured smooth and natural transitions

This case demonstrates the power of straight-line nose surgery using rib cartilage to transform a small, bulbous, and upturned nose into a structured, high-profile shape. By carefully considering the patient's skin limitations and desired profile, we were able to create a defined jikban-line that blends seamlessly with her face.

The use of autologous rib cartilage not only allowed for safe structural extension but also helped maintain long-term support and stability. Adjustments to the nasolabial angle played a crucial role in improving the patient’s side profile by softening the impression of a protruding mouth.

The final result? A beautifully elevated, straight-line nose that balances well with her entire facial structure.
