Hello, this is Dr. Chayoung Kang, Director of Noselab Rhinoplasty Clinic.

Today, we will introduce a corrective case of a patient with severe nostril asymmetry and nasal deviation.


This patient had undergone rhinoplasty at another hospital 2 years before but had not been able to achieve satisfactory results. She developed various issues, therefore she visited our hospital for Closed Rhinoplasty Korea correction.

Previous procedures and medical history:

  • 7~8 years prior: nose filler procedure (1 dissolving session)

  • 2 years and a half prior: rhinoplasty using donated rib cartilage and silicone at another hospital.

Frontal view:

  • Nose bent to one side

  • Exposed nostrils

  • Noticeable asymmetric nostrils

  • High nose bridge starting point


Side profile view:

  • High glabella starting point

  • Short and upturned nose tip

  • The mid-face appeared long.

  • The croocked nose is more prominent

  • Upturned nose tip

Surgical Plan with Functional & Cosmetic Focus

The patient's major complaints were regarding the existing breathing problems...

After determining the patient's condition and requirements, we developed the following plan:


  • Foreign body removal (filler)

  • Septal reconstruction using autologous rib cartilage

  • Septal extension graft

  • Septoplasty

  • Inferior turbinates submucosal resection using a high-frequency laser.

  • Achieved a more harmonious appearance overall by lowering and lenghtening the nose tip.

  • Significant improvement of the severly asymmetrical nostrils.

  • Straightening of the columella that was previously leaning to one side.

  • The narrow right nostril was adjusted to be wider and to achieve balance between both sides.

Overview

This complex rhinoplasty case achieved both aesthetic and functional improvements. The following are particularly noteworthy:

1. Removal of foreign substances

   By removing the existing filler and using silicone implants to smooth the nose shape, we can expect natural results.

2. Septal reconstruction and correction:

   By reconstructing and correcting the severely damaged and deviated nasal septum, we were able to secure the structural stability of the nose, and improve the respiratory function.

3. Improvment of the nostril asymmetry:

   Severe nostril asymmetry could be naturally improved through septal correction and alar cartilage repositioning.

4. Correction of the crooked nose tip:

   Through septal extension graft surgery, the tip of the nose, which was previously upturned, was lowered,at the same time that the midface was made to look shorter.

5. Improved respiratory function:

   The patient's major complaints were regarding the existing breathing problems. These were significantly improved through septoplasty and inferior turbinate resection surgery.

Conclusion

​When the tip of the nose becomes tilted to one side, especially if is related to severe septal deviation, if the deviation is not corrected, the tip will continue to bend to one side. This means that it is difficult to fundamentally resolve the problem by solely correcting the tip of the nose.

As can be seen in this case, the problems of nasal deviation and nostril asymmetry are often closely related to the problems of the nasal septum. Therefore, in order to solve these complex problems, it is important to understand the structure of the nose as a whole, find the root cause, and correct it.
