This patient presented after a previous rhinoplasty with an over-rotated nasal tip, a short columella, and an unnaturally rigid nasal bridge.
From the frontal view, excessive nostril exposure created an artificial appearance, while the side profile lacked balance due to unstable tip support.
In revision rhinoplasty, these problems are most commonly caused by insufficient columellar and septal support, leading to exaggerated tip rotation and an unnatural nasal base.

Surgical Plan
Septal Extension Using Autologous Rib Cartilage
In this Columella Extension Rhinoplasty Korea case, septal extension was performed using autologous rib cartilage.
