Surgical Background

(Columella Extension Rhinoplasty Korea)

This patient presented after a previous rhinoplasty with an over-rotated nasal tip, a short columella, and an unnaturally rigid nasal bridge.

From the frontal view, excessive nostril exposure created an artificial appearance, while the side profile lacked balance due to unstable tip support.

In revision rhinoplasty, these problems are most commonly caused by insufficient columellar and septal support, leading to exaggerated tip rotation and an unnatural nasal base.

Surgical Plan

Septal Extension Using Autologous Rib Cartilage

In this Columella Extension Rhinoplasty Korea case, septal extension was performed using autologous rib cartilage.

This method allows precise control of columella length, nasal tip projection, and rotation angle, while providing strong and long-lasting structural support—especially critical in revision cases.

Columella Lengthening and Tip Repositioning

The shortened columella was carefully lowered to restore proper nasal proportions.

This significantly reduced nostril show from the frontal view and created a soft, elegant nasal tip without overcorrection.

Artificial Implant Removal and Dorsal Line Revision

The existing silicone implant that caused an artificial and sharp bridge line was removed.

A custom-carved silicone implant was then designed to fit the patient’s nasal bone anatomy, creating a smooth, straight-slope dorsal profile.

Nasolabial Angle Refinement

The nasolabial angle between the upper lip and columella was refined to enhance lateral facial harmony and achieve a more sophisticated profile.

Surgical Results

  • Columella lengthened to a natural and proportionate position

  • Over-rotated nasal tip corrected to a stable angle

  • Excessive nostril exposure significantly reduced

  • Artificial bridge replaced with a soft, natural dorsal line

  • Balanced and refined appearance from all angles

Final Outcome:A feminine, natural-looking nose with long-term structural stability.

Surgeon’s Commentary

Columella extension is not merely lowering the nasal base—it is a comprehensive reconstruction of nasal support.

In revision rhinoplasty, structural stability must come before aesthetics.

Autologous rib cartilage allows precise control and long-term durability, making it ideal for septal extension in complex cases.

Overcorrection must be avoided; excessive lowering can cause a droopy or aged appearance.

At NoseLab Clinic, Columella Extension Rhinoplasty Korea is performed with one principle:

rebuild structure first, then refine aesthetics—never the other way around.
