[Nose Lab Clinic]

Hello, this is Dr. Cha-Young Kang, Director of Nose Lab Clinic.

Today, I’d like to share a challenging case of Gore-Tex Contracture Revision Rhinoplasty involving a 17-year-old nasal implant that caused contracture, severe nasal asymmetry, septal perforation, and chronic rhinitis. Through complex reconstructive surgery with autologous rib cartilage, we restored both function and aesthetics.

Surgical History – The Path to Gore-Tex Contracture Revision Rhinoplasty

  • 17 years ago: Gore-Tex nasal implant (bridge + tip) + Alar Base Reduction

  • 3–5 years ago: Septal perforation repair attempt + Non-implant revision

Initial Diagnosis & Key Issues

  • Persistent septal perforation

  • Severe septal cartilage loss

  • Nasal asymmetry due to contracture

  • Restricted nasal skin elasticity

  • Chronic rhinitis symptoms

Surgical Goals in Gore-Tex Contracture Revision Rhinoplasty

  • Close the septal perforation completely

  • Rebuild missing septal cartilage with autologous rib

  • Restore nasal symmetry & reduce contracture

  • Improve nasal airflow and relieve chronic rhinitis

Surgical Procedure

Key Intraoperative Findings

  • Septal perforation larger than anticipated

  • Severe depletion of septal cartilage

  • Extensive scar tissue & contracture

  • Thinned nasal mucosa from prolonged implant stress

Reconstruction Steps

  • Closed septal perforation using autologous mucosal graft

  • Rebuilt septal cartilage with rib cartilage graft

  • Removed scar tissue while preserving native structures

  • Corrected nostril asymmetry and refined alar base

Functional Enhancements

  • Treated chronic rhinitis with submucosal turbinate reduction

  • Reinforced nasal framework for long-term stability

Aesthetic Refinements

  • Corrected contracture-related deformities

  • Achieved natural nasal contour

  • Balanced nostril symmetry

Expert Insights on Gore-Tex Contracture Revision Rhinoplasty

This case highlights the complexities of long-term Gore-Tex implant complications.

Key Takeaways:

  • Gore-Tex implants can cause severe tissue contraction and cartilage absorption over time

  • Septal perforation repair requires precise grafting techniques

  • Autologous rib cartilage ensures lasting functional and aesthetic results
